Where this keyword comes from?

I have a ON-PAGE optimized website, but seomoz research tool says that 24 pages are F graded, and i was checking the keywords and i noticed that the keywords seomoz research tool was analising wasnt the ones that i used.
Let me explain its like this:
My Keyword is : Santa and seomoz is running the keyword : Santa Claus
Where did the tool got that Keyword from?

Hi Denis,
If I understand correctly, you have a PRO campaign that is grading a URL/Keyword combination that you didn't intend.
The On-Page tool automatically generates reports for any of your tracked keywords that rank in the top 50, and it grades them against the URL that is ranking. So generally, it can only get these keyword phrases from your tracked keywords.
Sometimes these are not the keyword/URL combinations you want to grade. Fortunately there's an easy way grade any keyword you want. Here's some instructions from the Help Guide I wrote...
For example, your "Contact Us" page ranks #5 for the keyword phrase "Yellow Shoes" and the Web App grades it a B. What if you really want to target your homepage for this keyword phase? Here’s how:
1. Hit **Report Card **at the top of the On-page summary.
2. Choose your Keyword you want to grade. The keyword must already be included in your campaign. Select Manage Keywords if you need to add keywords.
3. Enter the URL of the page you would like to grade.
4. Hit Grade My On-Page Optimization to generate your report.

Am I accidentally Keyword Stuffing?
Hey Guys, So I updated some copy on my site recently and noticed that whatever slender rankings I had (often on page 😎 have completely disappeared. The copy was the only change I have made. Now I haven't intentionally keyword stuffed however I have noticed that there happens to be a lot keywords in there. For example on my PPC page I use the phrase PPC 16 times however it has just naturally fallen into the content as that is what I'm writing about. I'm wondering if there are maybe too many mentions here? 16 repeats of the word PPC and on the page there are 490 words. Does that feel like too much repetition or am I barking up the wrong tree? What is the best way to deal with creating a separate brand with it's own website when the main site already ranks well for the target keywords?
A client currently has a site that ranks well for a number of queries. They recently created a new site for a spin-off brand/company that they now want to focus on ranking for some of the keywords their original site already ranked for. What would be the best way to go about this without throwing away the existing authority and traffic the original site has for those queries?

Multiple keywords over multiple domains - am I missing the point?
This I think, is a conceptual question related to Moz/ KeywordTracking in general. Q: What is a "good" way to setup tracking for keywords across many pages, potentially multiple domains? At present I've identified some keywords that are relevant to our products. That leads me to want to track not just for a specific page, but for their rankings across multiple pages, and potentially at least two domains. One site is our main product site, the other a blog/info site. This is I suppose mostly discovery at this point. Working out what, if any, of our pages are ranking for a full set of keywords that we believe are related to our products. It may be that I'm completely missing the point of tracking, that I'm not using it as intended. I want to learn how our pages track currently (for a bunch of keywords), see that change over time as we make changes, and also visualise what we're strong in and what we're not. To me, this would let me see just where the holes are in our SEO easily. The reason I ask is that it seems I have to manually enter a keyword plus a webpage in Moz. Given I've 20-30 keywords I want to track many pages, this is going to take me "quite some time" (tm). Is there a better way to do what I describe here? Am I missing the point of keyword tracking?

Is it difficult to rank for a keyword phrase with an H2 halfway down the page?
If your competitor has a page that is optimized for a keyword (title tag, URL, content) and you target the keyword halfway down a page with an H2 and a short paragraph about the keyword, are you at a strong disadvantage?

Popup windows are coming up as 404 error in moz reports.
We have several links showing up as 404 errors because of the way our site is set up. I want to know if this is hurting our ranking because Google sees it as 404 errors or is it just something I can ignore because it works for the user? If it is hurting our quality and therefore our rankings, how can I correct it so that best practices are used? I have attached an image of the links and here is an example page --> http://www.sourcemedicalequipment.com/Perch-Polyurethane-Industrial-Stool-18-25-p/idst2.htm The links in the description section have anchor text "All 3 Choices" and result in a popup information page. If you cut and paste the URL's that they are redirecting to end in a 404 but if you use the link on the page it results in a popup information window. Hope I explained that well. Strategy for dealing with keyword variants
I'd like your opinion on the strategy of dealing with key phrase variants: I've got a page that is ranking in the top 10 and top 3 for about 10 different variations of the same phrase. Seomoz gives it terrible grades for all but one of the keyphrases. Which course of action do you recommend : ----------- create new pages ------------ Create new pages for each variant and get them to grade A. If I go this route what sort of cross linking scheme should I use between this pages? ---------- improve existing page ----------- Try to make my single page rank better for all the phrases by doing extra on-page work so the page has a better grade for all of the variants? On page Analysis... how does seomoz choose to match a keyword to a url?
On my "On Page" analysis report it show me a couple pages that have D ratings... one key phrase is "my destination lodging" and it is matching it up with a url like mydomain.com/my-destination-hotels.html. It says I'm ranking 37th for this term.... Is it just suggesting that I optimize this url because it is already ranking for that term? So should I create a different page with a different url to better match this phrase? Thanks for any ideas.
