Issue Duplicate Page Title

I'm having some really strange issues with duplicate page titles and I can't seem to figure out what's going on. I just got a new crawl from SEOMOZ and it's showing some duplicate page titles.
http://www.example.com/blog/page/2/
http://www.example.com/blog/page/3/
Repeat ..............
I have no idea what's going on, how these were duplicated, or how to correct it. Does anyone have a chance to take a look and see if you can figure out what's happening and what I need to do to correct the errors? I'm using Wordpress and all in one SEO plugin.

Add the "noindex" tag to subpages of archive pages to fix the duplicate page title and potential duplicate content issues. Alternatively, you can add page numbers to you template (but that won't take care of the duplicate content problem).
This feature is available in Yoast's WordPress SEO plugin. I recommend migrating to it from All-In-One. AIO may have the same feature -- check the Settings panel.

We have around 17 pages have underscores in the URL. From the 17 pages, we have changed 3 pages URL for example if the url is test_sample_demo.html, we have changed as test-sample-demo.html After the updates, we have made redirect as follows Redirect 301 test_sample_demo.html test-sample-demo.html Presently google webmaster tool shows as "Pages with duplicate meta descriptions" & "Pages with duplicate title tags" for changed pages How to fix this. Hreflang and possible duplicate content SEO issue

So, on that German version of page, everything like menus, navigation and such are in German, but the video is the same, the title of the video (H1 tag) is the same, <title></code></strong> and <strong><code>meta description</code></strong> is the same as on the original English page. It means that general (English) page and German version of it has the same key content in English.</p> <p>To me it seems to be a SEO duplicate content issue. Duplicate pages on wordpress
I am doing SEO on a site which is running on WP. And it has all pages and categories duplicates on domain.com/site/ However, as it got crawled I saw that all domain.com/ pages have rel=canonical with main page tag (does it mean something?). Thing is I will fix permalinks structure and I think WP automatically redirects if it is changed from /?page_id= to /%category%/%postname%/ or /%postname%/ Isn't there something I miss? Second problems is a forum. After a crawl it found over 5k errors and over 5k warnings. Those are: Duplicate page content; Duplicate page title; Overly-Dynamic URLs; Missing Meta descr; Title Element too long. All those come from domain.com/forum/ (fortunately, there are no domain.com/site/forum duplicates). What could be an easy solution to this?

Duplicate Page Title Crawl Error Issue
In the last crawl for on of our client websites the duplicate page title and page content numbers were very high. They are reading every page twice. http://www.barefootparadisevacations.com and http://barefootparadisevacations.com are being read as two different pages with the same page title. After the last crawl I used our built in redirect tool to redirect the urls, but the most recent crawl showed the same issue. Is this issue really hurting our rankings and if so, any suggestions on a fix for the problem? Thank you!

Changing title tag in wordpress media pages
Hello! I have a problem of duplicate title on 59 pages in Worpress. I Guess it happened after a recent WP update, because, suddenly, I got a peak of 59 from zero in a day. The SeoMoz crawl report states that there are 59 duplicate title. As you may see in the picture those are all media pages, whose title is written really badly with the formula postname/filename/blogTitle, ending up with trunkated, too long, title tags, that result in duplicate page title. How can I simply hide these mediapages or sculpting the title tag the way I want? I am using all-in-one-seo WP plugin, which doesn't seem to provide a solution. Thank you all! Duplicate Content Issue
Very strange issue I noticed today. In my SEOMoz Campaigns I noticed thousands of Warnings and Errors! I noticed that any page on my website ending in .php can be duplicated by adding anything you want to the end of the url, which seems to be causing these issues. Ex: Normal URL - www.example.com/testing.php Duplicate URL - www.example.com/testing.php/helloworld The duplicate URL displays the page without the images, but all the text and information is present, duplicating the Normal page. I Also found that many of my PDFs seemed to be getting duplicated burried in directories after directories, which I never ever put in place. Ex: www.example.com/catalog/pdfs/testing.pdf/pdfs/another.pdf/pdfs/more.pdfs/pdfs/ ... when the pdfs are only located in a pdfs directory! I am very confused on how to fix this problem. Maybe with some sort of redirect?
