Duplicate page errors from pages don't even exist

Hi,
I am having this issue within SEOmoz's Crawl Diagnosis report. There are a lot of crawl errors happening with pages don't even exist.
My website has around 40-50 pages but SEO report shows that 375 pages have been crawled.
My guess is that the errors have something to do with my recent htaccess configuration. I recently configured my htaccess to add trailing slash at the end of URLs. There is no internal linking issue such as infinite loop when navigating the website but the looping is reported in the SEOmoz's report.

btw there is no issue such as crawl error in my Google webmaster tool.

Thanks Mohammad. I am glad it is all resolved. Yes, broken or malformed links can often be like gremlins and cause all kinds of odd things to show up in crawl reports.

Hi Mohammed,
Are you using absolute URLs for any canonical tags you have in place? Also, are you using absolute URLs for all of your internal links? You probably are, but I thought I'd better ask before diving in deeper. Hope I can help a little.

I can't crawl the archive of this website with Screaming Frog
Hi I'm trying to crawl this website (http://zeri.info/) with Screaming Frog but because of some technical issue with their site (i can't find what is causing it) i'm able to crawl only the first page of each category (ex. http://zeri.info/sport/) and then it will go to crawl each page of their archive (hundreds of thousands of pages) but it won't crawl the links inside these pages. Pages appear fine in browser but 404 error when crawled?
I am working on an eCommerce website that has been written in WordPress with the shop pages in E commerce Plus PHP v6.2.7. All the shop product pages appear to work fine in a browser but 404 errors are returned when the pages are crawled. WMT also returns a 404 error when ‘fetch as Google’ is used. The word 'shop' in a page title
I'm reworking most of the page titles on our site and I'm considering the use of the word 'Shop' before a product category. ex. Shop 'keyword' | Brand Name As opposed to just using the keyword sans 'Shop.' Some of the keywords are very generic, especially for a top level category page. Question: Is the word 'Shop' damaging my SEO efforts in any way?

Error: Missing Meta Description Tag on pages I can't find in order to correct
This seems silly, but I have errors on blog URLs in our WordPress site that I don't know how to access because they are not in our Dashboard. We are using All in One SEO. The errors are for blog archive dates, authors and just simply 'blog'. Here are samples: http://www.fateyes.com/2012/10/

Why do I get duplicate page title errors.
I keep getting duplicate page title errors on www.etraxc.com/ and www.etraxc.com/default.asp, which are both pointing to the same page. How do i resolve this and how bad is it hurting my SEO.

Duplicate Page Content and Titles
A few weeks ago my error count went up for Duplicate Page Content and Titles. 4 errors in all. A week later the errors were gone... But now they are back. I made changes to the Webconfig over a month ago but nothing since. Why do I have one page showing as two url's?
My SEOMoz stats show that I have duplicate titles for the following two url's: http://www.rmtracking.com/products.php and http://www.rmtracking.com/products I have checked my server files, and I don't see a live page without the php. A while back, we converted our site from html to php, but the html pages have 301's and as you can see the page without the php is properly redirecting to the php page. Any ideas why this would show as two separate url's?
