My website is coming up under a proxy server "HideMyAss.com." How do I stop this from happening?

We've noticed that when we search our web copy in Google the first result is under a proxy server "HideMyAss.com," and our actual website is no where in sight. We've called Google and they really didn't have an answer for us (well the 2-3 people) we spoke with.
Any suggestions or ideas would be greatly appreciated.

That's very frustrating! I've never had a problem with HMA specifically, but that script usually works.
You may want to try searching for things like "break out of frame proxy" or "break out of frame php" to see if anyone out there has come up with a better solution.

We've added the break out of frame script but it doesn't work. Hidemyass is too smart for that script.
We also tried disallowing access proxy servers via PHP (see below) and htaccess. For PHP, it also disallow some pages under CDN. We might try turning off CDN and try PHP again. Htaccess doesn't work at all for hidemyass.

Great call on both the canonical tags and breaking out of the frame. I imagine the site will be indexed properly within a week using these tactics.

Interesting. Google is usually smart about not indexing proxies, but here it looks like it's doing exactly that.
You can try using Javascript to break your site out of any frames when a visiting your site. This will break the user out of the frame if they come via the proxy, and may also help prevent Google from indexing the proxy frame:

It's when we copy some web copy into Google. For instance if you enter this text into Google it will come up "At American Addiction Centers, treatment programs are tailored to meet the precise goals and needs of each individual. Our staff develops working relationships with clients while reviving mind, body and spirit."
And that's true for most of our web content.

We have the canonical tags on all our headers. And there are no manual penalties listed in webmaster tools. Any thoughts?

Make sure you have the canonical tag in the header of all your pages. It helps Google tell what is the original source of a page:
https://support.google.com/webmasters/answer/139394?hl=en
Also, check in webmaster tools to see if your site has a manual penalty, being penalized can cause other sites to show up higher than you in the SERPs for your content.

Canonical tags being direct to "page=all" pages for an Ecommerce website
I find it alarming that my client has canonical tags pointing to "page=all" product gallery pages. Some of these product gallery pages have over 100 products and I think this could effect load time, especially for mobile. I would like to get some insight from the community on this, thanks!

Update: Copied Website
So I discovered a website the other day that is a complete duplicate of ours: justinchina.co.uk This is our website: petmedicalcenter.com . Thanks to help from Erica, I dug in deeper to see why this was happening. It seems that the justinchinca.co.uk which is hosted by GoDaddy has their A Record pointing at our web host. So that being said, our website does not seem to be hacked which is good news. Would this still cause an issue with our Google rankings? Our host, Host Monster said to contact GoDaddy and GoDaddy said that a domain owner can point their URL to anywhere that they choose. Anyway, any feedback would be helpful. What is Happening to Me?!
Okay so this morning I logged into webmaster tools and saw this notice of manual penalty for "unnatural links" affecting "some incoming links." (see attached photo) This is a different notification than I've seen before. Keep in mind this website had been building unnatural links up until I took over in December of 2012. I have since cleaned up a TON of them and thought I was making progress... Until this morning that is. So the next thing I did was head over to the incoming links in GWT. What I saw here shocked me. Whereas the last time I checked (maybe 2 days ago) we had approx 6,000 links we now had 195,565 !!! WTF?!?! Okay so somebody is screwing with us, right? But here's the real confusion: I can't find any of these links anywhere else other than GWT. As you can see on the other attachment I have included, Google is reporting some 39,000 links from a domain called "itbriefing.net" however when I search this domain I find absolutely zero of these links. There are a few mentions of our brand but not one single link. Also, looking through ahrefs.com and OSE I don't see any of the data that GWT is reporting here; everything looks normal to me. Now isn't GWT the last to report all of these changes? What could possibly be happening here? Am I losing my mind? We are still being indexed although I've been frustrated trying to get one of our main keywords to re-rank. This was one that had the most poison links built over the years so recently I gave up and changed the URL of the page completely sans redirect. This worked, and GWT reported a ton of lost links (99% of which were poison and the other 1% of which I regained with the new URL). But now this. Is this a GWT fluke? Am I under attack? Can you disavow a spamy link that is not pointing to your website?
We have submitted several really spammy websites to the Google spam team. We noticed they take a very long time to react to submissions. Do you know if it is possible to disavow a link that is not pointing to your website but rather to a very spammy website? Thanks

New website :301 redirection of a established domain
Hello , I am launching a new website which would host user generated content . Based on my brandname i have purchased a new domain . In order to improve SEO rankings i was considering to purchase a good quality domain (have gr8 link backs) and then perform 301 redirection of the domain to the new brandname.co.in domain .
