What would Cause listing to fall off local search map spot?

Any reason a listing that was showing in Google between the 3 and 5 spot on local map search would suddenly disappear all together from the map position for a specific keyword?

Yes very weird, it came back even improved a spot or two a couple of time and now has been missing for a little while. Not used to seeing something always in the number 4 spot completely dis sapper.
Only thing that I have done is straightened out his directories with Moz Local which should help but YP still has a tracking number on his listing which they always did and claim that they have a way for Google to read the real number on the backend but show the public the tracking.
Not sure I believe that. Even went as far to have his YP call me when I was trying to get him to have them put it back to his real number. Don't think this would cause this though.
This is just for keyword Dentist St Louis... He still shows good for Cosmetic Dentist St Louis and Emergency Dentist St Louis

Losing rankings in this manner tends to stem from 1 of 3 things:

An algo change on Google's part
-
Being surpassed by more active competitors
-
A mistake on the business owner's part with negative consequences
I recommend that you start troubleshooting here:

That should help to rule out any obvious mistakes. Other issues to investigate right now might be whether your competitors have ramped up their game plan and also, if you are in the entertainment industries, researching how the recent removal of the Carousel has altered the SERPs would be important.

Fighting spam on Google Maps
"Suggest an edit" on Google Maps works occasionally and so does Google's Redressal Form. Sending a DM to @GoogleMyBiz on Twitter does too. But it seems like the vast majority of spammy businesses that I report, go unnoticed by Google. I'd really appreciate hearing from some other SEOs on how they remove crap from the map, with better results. Thanks.

Local Ranking with No Physical Address in New Service Area - How to Rank?
OK, SO, I am a wedding company in Maui, Hawaii and have an established business on one island with a physical address. http://simplemauiwedding.net We have started a new team in Oahu, Hawaii http://simpleoahuwedding.com and we provide service there and have a full team in place. Business Name Not Showing Up in Google's Maps
I have a client whose name in not currently showing up on Google maps. Their business location only shows once their name or related keywords are searched, but their business name does not show when you only look for it on the map regardless of how far zoomed in you are to the actual location. I am wondering if anyone else has experienced this, or knows of a way to fix this. I have already contacted Google multiple times, and they told me that “business’ names are just randomly pulled”. The client is an HVAC store front business with good rankings and a fully optimized Google profile, so these reasons did not answer the issue. Do you need contact details (NAP) on every page of your website for local search ranking ?
We’ve got a clients site which doesn't have the contact details on every page, all the contact details are on the /contact page which is using the schema.org local business markup Some sites that our outranking us locally have their contact details on all pages, where as others only have it on the contact page also. Is having your contact details on every page a ranking factor for local search ?

No Location option in Incognito Search Settings
I was checking on a client ranking and went to Incognito in Chrome for the search. I went to search settings to set the location and thought I had done something wrong. I closed and went back to search settings and still no location setting. See attached. Interestingly, when I went to my signed in Chrome and set the location and then went to incognito and went to search settings, then location showed up for me. Is eLocal a scam or legitimate directory for local SEO?
I just got an email from eLocal with information that is way farther off than any other email I have received from directories I know. I ran a search on them, and it definitely seems fishy. Plus, it's not showing up as a problem in my Moz Local account. However, I don't want an inconsistent listing if this is a legitimate site I should correct. Anyone have experience with them? PO Box for a Local Client
I have a local client who is adamant about not publishing their address anywhere online. They are a local (regional) company, and work out of their home. I advised that PO boxes do not fill within "accurate guidelines" for Google Places (and moving forward any citation building is going to be tough). Any way to get around this besides buying a generic mailbox that isn't a PO Box? They feel putting their home address anywhere leaves them vulnerable to people showing up at their home, and considering the industry they are in, it is possible this could lead to negative confrontations and I understand their concern. Any ideas for me?
