Would you recommend changing image file names retroactively?

We'll be optimizing file names moving forward, but it hasn't been done in the past. I have probably 1,500 that would need to be changed, so want to make sure it's a valuable use of time/worth the effort. Also, has anyone used Media File Renamer for wordpress? Seems this would be a good way to rename files without needing to reupload everything. Thanks!

Generally speaking, in our experience, it's not worth it to take the time to change filenames (and thus change URLs) unless you absolutely have to. If you're still using the same CMS, then just changing the URL might not be worth it, as you have to take time to set up a 301 redirect from the old URL to the new URL.
What it would be worth it to do is to go with a flat file structure, such as domain.com/category/page or domain.com/page/. In the long run, you won't generally have to change URLs in the future if you move to another CMS.
If those pages are ranking well with the current URL, you may not want to change the filename. But, if they aren't ranking on the first page, it should be fine to change the filename. Don't forget to set up 301 redirects from the old URL to the new URL.

Practice Name vs. Dr. Name in Local Search
I wanted to get some opinions on an interesting situation for local search. Many doctors and dentists are found in one of two ways online: Their name and title, e.g. Dr. James A. Smith, MD Their practice name, e.g Smith Plastic Surgery, P.C Often sites like Healthgrades are providing data on the doctor him or herself, and the information can be tough to switch out. At the same time, there's a tendancy on the Dr's part to want to be listed as their practice name. Their site is often set up that way. How are you handling this kind of setup? Have you found a way to reliably associate (and format) a practice and doctor's name in a listing, in such a way that doesn't violate Google's quality guidelines? I know the drill for handling a doctor within a hospital, but this is a slightly different situation...

Hey guys. When targeting a specific keyword that mentions a city (eg. Dover Used Cars) would it be a good practice to include that city in the URL when creating additional pages (eg. placethatsellsusedcars.com/dover-used-fords)? Thanks!
