Google Disavow File Update

Is there any specific format to update the Disavow file? Also if I submitted the file a months ago, and need to update it now... should I leave the old 'excluded domains' or should I remove them?
Lets say this is what I have: How would you update it?

Yes. I did keep in most of the old domains that had been in the original Disavow file.
Since the things I learned after doing the original file showed that I had mistakenly disavowed a few domains that were OK, I did omit them from the new file. And I added some that had not been in the original.
Nobody but Google knows for sure, but my understanding is that a particular Disavow only has an effect while that particular file is active on your Google Webmaster account. So if you delete it or upload a new one, then you will eventually see the old effect disappear and be replaced by the effect of having no or a different disavow file.
I just wish the effect wasn't so slow on Google. The effect of disavowing linking domains in Bing Webmaster Tools seems to be very quick. But it's rarely needed there since Bing doesn't have nasty Pandas and Penguins to battle...

When you Re-Submitted the file, did you include the old domains? I am unsure as to whether or not keep the original domains into the updated file, as i haven't read anything against it or in favor for such method.

Hi Daniel,
I also submitted a Disavow file, then learned new things and updated the file to reflect the new knowledge.
You can only have ONE disavow file. So you simply create a new file (which may or may not contain many domains that were in the original) and then go to Google Webmaster Tools and delete the original file and upload the new one.

Especially this part from the article:
Google rejects many disavow files because of bad formatting, but webmasters usually never know. Guidelines state the file type should be .txt only and “must be encoded UTF-8 or 7-bit ASCII.”

I found these because Moz showed these sites in my list of backlinks. I assume that means it impacts my Domain Authority. Should you do a disavow even if you don't have a manual penalty?
If you are working on a website which has a history spammy links, but no manual penalty by Google... is it still worthwhile to still go through the link removal and disavow process? Thoughts appreciated.

Submitting Separate Disavow Files for Each Subdomain?
This question has to do with using Google's Disavow Links Tool. For sites having quite a few subdomains, where those subdomains are listed separately in Google's Webmaster Tools, do I have to upload a separate disavow file for each subdomain? Or, will the file that I upload for the main domain (root domain), also be evaluated by Google for all of the subdomains on that site? I've searched all over the Internet for an answer to this question, but can't find anything.

Last Linkscape index update
I noticed that under my link analysis that it said: "Last Linkscape index update: 08/14/2012." I would like to try to get a full understanding of what this means? Directory submission after Google panda update
I am very much concerned about directory submission after Google panda update. I have few questions in this regard 1- Should I submit the home page URL or deep links in directories? Google Cache Date
The Google Cache date for my website: This is Google's cache of http://www.petmedicalcenter.com/. It is a snapshot of the page as it appeared on Aug 28, 2011 03:19:05 GMT. Does this date coorelate to the last time Google reevaluated their rankings for my site? So if I had done 15 or so backlinks after the August 28 date, will those start to affect rankings (once they are discovered) after the next time the site is updated?

Why does opensiteexplorer tell me I have 292 linking domains and google webmasteer tools says 528?
Does anyone have any idea why opensiteexplorer has a specific number of linking domains while google webmaster tools say another? I understand opensiteexplorer needs to update itself, but I have seen this discrepancy for months! Any ideas?
