Personally I would say this is a branding issue, if your logo looks like that then try to incorporate it within your links however do you think a customer would write it like that? I don't think this is an SEO issue, it's a branding question.

Personally I would do the structure you have mentioned above but refrain from using the geo targeting tool but it could get messy in the event of having UK and US versions?

If it's not manual (from Google Webmaster Tools) and the website remains unchanged and the drop was sudden then this will be an algorithm punishment. Or the industry has become more saturated, how did you generate backlinks to the website? was there any on going SEO efforts? what time did it drop?

Hi,
Firstly it could be that Open Site Explorer has not had time to pick them up, if it's years old then let's not forget the OSE manually crawls the internet and is not Google's crawler meaning there will be sections of the web which won't be crawled by Moz.
If it's old then there may be no links pointing to these pages which would not give OSE a reason to crawl them, certainly if it's years. Don't worry about it, a missing link on OSE is one less your competitors can find
I had a similar issue with a few links from BOTW but in time it updated and works fine but Open Site Explorer is not flawless.
