If I were you I would approach it not using exact match anchor text and not interlinking the 5 sites together. Build more of a pyramid when linking multiple sites to a money site. If you provide me a little more detail i can provide more assistance.

Agreed. A dedicated landing page on the same domain is the best soultion. Make sure to target variations of the keyword and location. Keep in mind that most of organic search traffic will be generated from the local results. Here is a little secret in ranking in neighboring cities for local results even if you do not have a physical address there.
- Find a local office building that rents out suite numbers. Ask to purchase the mailbox associated with an empty suite. Most office buildings rent their vacant suite mailboxes for $20 a month, Fill out a google places profile reflecting this new location in a neighboring town and BAM! You will now rank in local results in you neighboring town.
Make sure not to spam you local listing and build quality citations from local business around your area.
Press Releases really help as well! Hope this helps.

That is correct a 301 Redirect pushes all existing link equity to the new url structure. in order to prevent bad link equity to push through, do not 301 an old url to the new url.

Honestly Luke, don't worry about keyword density and the number of times a keyword (or how many keywords) you are targeting/using per page. In my opinion I typically only target 1 main keyword and a couple variations of the keywords. Dont get hung up on using it a certain % of time. Make sure your content is written well, informative, and helps increase conversions. Google is really starting to crack the whip.
My rule of thumb is this:
Title: Leading Plastic Surgeon in Seattle, WA | Dr. John Smith Plastic Surgery
Meta Description: Dr. John Smith specialize is an awesome plasitc sugeon who can help you acheive your aesthetic goals.
Keyword Tags -DO NOT USE
H1: Plastic Surgeon in Seattle, Wa
H2: How much does Plastic Surgery Cost?
Body of Content add location maybe once and write good 100% unique content.
