If I had to make the call, I'd 301 redirect the relevant old pages to the corresponding new pages.  I'd also tell Google that you've updated your web address using GWT.  Obviously there's not much you can do with the new pages that don't have relevant old pages, so just leave those to be indexed as normal.

I'm doing a routine clean up of my code and had a question about the canonical tag.

On the index page, I have the following:

I have never put any thought into which index path is the best to use.

http://www.example.com

http://www.example.com/

http://www.example.com/index.php

Could someone shed some light on this for me?  Does it make a difference?
