Have you checked the google keyword external tool to get an idea of the # of searches per month you get on certain combinations of keywords? That will help you decide the order of the keywords.
For example you might find out that more people search for "Photographer Colorado Springs"
Joel is correct. Analytics does still allow you to download a report that shows the actual keywords used that led them to your website
I have a site that gets pretty decent rankings. Based on Seomoz's assessment of my site I have ALOT of title tags that are too long.
If I make modifications to my title tags will that hurt my rankings?
I am also thinking of removing my company name from the title tag as that is taking up too much space.
Has anyone else heard of Google penalizing websites for having their contact forms located at the top of the website?

As long as you don't have one key word anchor text that just dominates the others then you should be fine however best practices are to try and incorporate plain urls for some of the links. For example it wouldn't look natural if you had one keyword that appeared 100 times while the others only appeared 5 or 10 times
As you are already doing It's a good idea to spread the links around to different pages and use different anchor text if as long as the anchor text is related to that particular page.
You are only penalized when you have duplicate content that resides on your website. For example if you have blog post on your website and that same post exists on another website. ..........that is duplicate content and you could be penalized.
If you are guest posting and using the same content for each one the owner of the website that you are posting on needs to be concerned with that. It's a good idea for a website owner to run content through copyscape before agreeing to post it on their website.

It looks like you have A LOT of out bound links (internal) from the home page. That can dilute the PR vaule of that page since you are spreading out your link juice to a ton of interal pages.
What kind of weekly or monthly traffic are you getting right now? Has it increased or decreased over the last 6 months?
What Keyword(s) are you targeting and where do you currently rank for those? Have the rankings gone up or down?
