URL rewriting causing problems

I am having problems with my URL rewriting to create seo friendly / user friendly URL's. I hope you follow me as I try to explain what is happening...
Since the creation of my rewrite rule I am getting lots of errors in my SEOMOZ report and Google WMT reports due to duplicate content, titles, description etc
For example for a product detail, it takes the page and instead of a URL parameter it creates a user friendly url of
mydomain.com/games-playstation-vita-psp/B0054QAS
However in the google index there is also the following friendly URL which is the same page - which I would like to remove
domain.com/games-playstation-vita/B0054QAS
The key to the rewrite on the above URLs is the /B0054QAS appended at the end - this tells the script which product to load, the details preceeding this could be in effect rubbish
i.e. domain.com/a-load-of-rubbish/B0054QAS and it would still bring back the same page as above.
What is the best way of resolving the duplicate URLs that are currently in the google index which is causing problems
The same issue is causing a quite serious a 5XX error on one of the generated URLs

, if I click on the link the link does work - it takes you to the retailers site, but again it is the number appended at the end that is the key - the retailersname is just there for user friendly search reasons
How can I block this or remove it from the results?

You can write canonical tags dynamically, but you need to canonical to the product specific page. Obviously, don't canonical every details.php page down to one. It could look something like this:
$prodDesc = "games-playstation-vita";
$prodCode = "B0054QAS";
$prodURL = "http://www.mydomain.com/".prodDesc."/".prodCode;
?>
I assume that the product description and code are generated from a database, so they should be available somehow to the header.

This is really tough to tell from generic examples, because the first thing you need to get at is the root of why these different URLs are being crawled in the first place. If "B0054QAS" is a unique product, why is it reachable by "playstation-vita", "playstation-vita-psp", etc.? That architectural problem has to be solved first, or any canonicalization is just a band-aid.

How would using the canonical tag work though - because I am using URL rewriting?
For example
mydomain.com/games-playstation-vita-psp/B0054QAS
is acutally something like details.php?=B0054QAS - so how would a canonical tag work in a dynamic template like above.
Because if I placed the tag below in the details.php page then I would need this to change dynamically based on the product - how can this be done?
http://www.mydomain.com/games-playstation-vita-psp/B0054QAS/>
Or would a 301 redirect be better to achieve the above?

To work on the duplicate problem, i think the best option is the cannonical tag. You will need to include in every page (pages you want to be the official one) inside the head tag the follow:
where ww.example.com/index.html is your page adress.
Than, to block old pages that you don't want to be at google, you can use the robots.txt file. There you will use the disallow function.

URL Change, Old URLs Still In Index
Recently changed URLs on a website to remove dynamic parameters. We 301'd the old dynamic links (canonical version) to the cleaner parameter-free URLs. We then updated the canonical tags to reflect these changes. All pages dropped at least a few ranking positions and now Moz shows both the new page ranking slightly lower in results pages and the old page still in the index. I feel like I'm splitting value between the two page versions until the old one disappears... is there a way to consolidate this quickly?

Some URLs in the sitemap not indexed
Our company site has hundreds of thousands of pages. Yet no matter how big or small the total page count, I have found that the "URLs Indexed" in GWMT has never matched "URLS in Sitemap". When we were small and now that we have a LOT more pages, there is always a discrepancy of ~10% or so missing from the index. It's difficult to know which pages are not indexed, but I have found some that I can verify are in the Sitemap.xml file but not at all in the index. When I go to GWMT I can "Fetch and Render" missing pages fine - it's not as though it's blocked or inaccessible. Any ideas on why this is? Is this type of discrepancy typical?

Magento URL change
We have a Magento website parked at HostGator. The site is comprised of both a PC and a mobile version. We changed the URL to a new one ... We made the domain changes in the ‘core_config_data’ (phpMyAdmin) ... We flushed the cache in the ‘File Manager’ part of cPanel (regular and mobile version) Currently we can access the http://newsite.com (on a desktop) with no problem ... We can also access http://m.newsite.com BUT… only from a desktop PC. When we try http://newsite.com from a MOBILE device, it routes to: http://m.OLDsite.com (it keeps going to the old URL) Need some help please. Thanks in advance!

Should I change the URL now?
Hi all, I have a client website that got hit in the latest algorithm update. It since appears that it had over 100 suspect links to it. I performed the Disavow procedure a few weeks ago via my Google Webmaster account, but have not received a message yet to say its been actioned. The majority of these suspect links go to one page. I am considering changing the base category (in Wordpress) to a different keyphrase and then submitting a new sitemap for indexing. This way there will be no actual link from a suspect website to a page on my website. Do you see what I mean? Will this help do you think? Thanks in advance.

Removing URL Parentheses in HTACCESS
Im reworking a website for a client, and their current URLs have parentheses. I'd like to get rid of these, but individual 301 redirects in htaccess is not practical, since the parentheses are located in many URLs. Does anyone know an HTACCESS rule that will simply remove URL parantheses as a 301 redirect?

Determine the best URL structure
Hi guys, I'm working my way through a URL restructure at the moment and I've several ideas about the best way to do it. However, it would be good to get some views on this. At the moment I'm working on a property website - http://bit.ly/N7eew7 As you can quickly see, the URL structure of the site needs a lot of work. Similar websites - http://bit.ly/WXH5WG http://bit.ly/Q3UiLC One of the sites has http://www.domain.ie/property-to-let/location/ And the other has http://www.domain.ie/rentals/location/property-to-let/ I could do with some guidance about the best steps to take with this. New URL structure caused a HUGE drop?
I have started working with a client who did an upgrade on their e-commerce sive in May of last year. It totally changed the URL structure and they didn't redirect old URLs or do any of the things they should have. Not unexpectedly they they went from about 300 visitors a day to 0 for then rose up to maybe 50 and have remained there ever since. There were some major onsite issues including about 15000 internal links that 302 back to the site. In any case I have fixed most of the onsite problems and worked on a little better categorization + content optimization, etc. We have only been working on this for about 30 days and organic traffic is up and they are ranking for much better keywords, but I expected a little quicker rise. Here is a screenshot out of GA of their descent. Its pretty rapid. I dont think it makes sense to redirect their old URLs at this point since most of them have been deindexed for 10+ months. Anyone have any suggestions on how to get back to their previous level. The domain actually has decent authority and link profile, etc. Is this just going to be a slow climb back? Any thoughts?
