Old URL showing up in SERPs 4 months after Re-direct

Hi guys,
I did a full site redirect back in October to a new URL, SERPS eventually changed to the new URL and everything was fine.
However recently i have started to see the old URL showing up?
Anyone else seeing this?

A few people have mentioned a refresh, supposed i'll have to wait and see.

This may (and I stress may) have something to do with whatever algorithmic update/refresh that many have experienced since the 17th. I've seen before with refreshers that old URLs pop back into SERPs, only for them to correct themselves a few days later.
Of course, just to be sure the 301 has not broken, click through on the link that's appearing to see if it takes you to the new landing page.
Given a few days, I imagine you'll see the proper URL displayed again.

Huge httaccess with old 301 redirects. Is it safe to delete all redirects with no traffic in last 2 months?
We have a huge httaccess file over several MB which seems to be the cause for slow server response time. There are lots of 301 redirects related to site migration from 9 months ago where all old URLs were redirected to new URL and also lots of 301 redirects from URL changes accumulated over the last 15 years. Is it safe to delete all 301 redirects which did not receive any traffic in last 2 months ? Or would you apply another criteria for identifying those 301 that can be safely deleted? Any way to get in google analytics or webmaster tools all 301 that received traffic in the last 2 months or any other easy way to identify those, apart from checking the apache log files ?

DMOZ snippet in SERP
I just added the author and publisher tags to my client's site and posted a dance class announcement on Google + with hashtags. When I did a test search on Google, they were showing up above the Place listings and the description under the title was taken from their DMOZ listing, not from the web site description. This is the first time I have seen this. Has anyone else seen DMOZ descriptions showing up in Google SERPs?

Can multiple redirects from old URLs hurt SEO?
We have a client that had an existing site with existing rankings. We rebuilt the site using DNN 7 and created/tested 301 redirects from all the Original URLs to the new DNN URLs which are nasty and have /tabid/1234 and will not allow for dashes (-)'s We have found a DNN module that will make the DNN 7 URLs search friendly. However, that will cause us to 301 the current DNN urls to the new URLs so in fact the original will redirect to the DNN and the DNN will redirect to the rewritten SEO friendly URLs. What should we know here before proceeding?

International URL Puzzle
Hello, I have 4 different URL's going to 4 different countries that all contain the same content and Google is seeing them as duplicate pages. For ecommerce reasons I have to have these 4 pages separated. Here is a example of the pages below so you can see the URL structure: www.example/com/canada www.example.com/australia www.example.com/usa www.example.com/UK How do I fix this duplicate content problem? Serps drop on 21st
Hi, This is my website: http://goo.gl/fl5a5 Earlier i used to be on #1 and #3 page results for most of my results. But after 21st dec, my results went to 10th and 13th page of results. Should I change wordpress urls?
Should I change my wordpress permalinks to include the keyword? For examples at the minute my url is http://www.musicliveuk.com/home/wedding-singer. Is it better to be http://www.musicliveuk.com/live-bands/wedding-singer. 'home' is not relevant so surely 'live-bands' would be better? If I change the urls won't I lose 'link juice' as external links will all point to a url that no longer exists? Or will wordpress automatically redirect the old url to the new one? Finally, if I should change the url as described how do I do it on wordpress? I can only see how to edit the last bit of the url and not the middle bit.

Changing a url from .html to .com
Hello, I have a client that has a site with a .html plugin and I have read that its best to not have this. We currently have pages ranking with this .html plug in. However If we take the plug in out will we lose rankings? would we need a 301 or something?

How to fix duplicated urls
I have an issue with duplicated pages. Should I use cannonical tag and if so, how? Or should change the page titles? This is causing my pages to compete with each other in the SERPs.
