Homepage Index vs Home vs Default?

Should your home page be www.yoursite.com/index.htm or home.htm or default.htm on an apache server? Someone asked me this, and I have no idea. On our wordpress site, I have never even seen this come up, but according to my friend, every homepage HAS to be one of those three. So my question is which one is best for an apache server site AND does it actually have to be one of those three?

If you want more information yes Apache server does make
Most Apache web servers have set the following pages as the default page with a directory.
default.htm default.html index.htm index.html index.php
If using WordPress you could see index.php because WordPress
Uses a PHP Database regardless of what your friend told you ideally you should have
Properly 301 redirected static links when talking about links that are going to be seen by Google.

Means WordPress websites ideally should not contain publicly visible index file of any kind.

Your friend is speaking about a 100% HTML site and the answer is your site should not end using
www.yoursite.com/index.htm or home.htm or default.htm
Using Word press your site should end in www.yoursite.com or www.yoursite.com/
Some people when they ring there site over from an HTML static site to WordPress they might find a parma links

If you do this and 301 redirect any links that would be changed by using WordPress to the new Permalink structure shown below or if you feel like using one of the others which I recommend against you may. However most sites are best served using the settings below.

How long for google to de-index old pages on my site?
I launched my redesigned website 4 days ago. I submitted a new site map, as well as submitted it to index in search console (google webmasters). I see that when I google my site, My new open graph settings are coming up correct. Still, a lot of my old site pages are definitely still indexed within google. How long will it take for google to drop off or "de-index" my old pages? Due to the way I restructured my website, a lot of the items are no longer available on my site. This is on purpose. I'm a graphic designer, and with the new change, I removed many old portfolio items, as well as any references to web design since I will no longer offering that service. My site is the following:

Site not indexed on Google UK after 4 days?

Wonder if anyone can help with this one? I have an ecommerce site www.doggydazzles.co.uk which went live on Friday and was submitted to Google via webmaster tools on saturday morning, but I can't find any trace of it in a google search?
I'm a bit stuck with this as its never happened to any of my other sites.

Google removing pages from Index for Panda effected sites?
We have several clients that we took over from other SEO firms in the last 6 months. We are seeing an odd trend. Links are disappearing from the reports. Not just the SEOmoz reports, but all the back link reports we use. Also... sites that pre Panda would show up as a citation or link, have not been showing up. Many are these are not Indexed, and are on large common Y.P or other type sites. Any one think Google is removing pages from the Index on sites based on Panda. Yours in all curiosity. PS ( we are not large enough to produce quantity data on this.)

Local SEO url format & structure: ".com/albany-tummy-tuck" vs ".com/tummy-tuck" vs ".com/procedures/tummy-tuck-albany-ny" etc."
We have a relatively new site (re: August '10) for a plastic surgeon who opened his own solo practice after 25+ years with a large group. Our current url structure goes 3 folders deep to arrive at our tummy tuck procedure landing page. The site architecture is solid and each plastic surgery procedure page (e.g. rhinoplasty, liposuction, facelift, etc.) is no more than a couple clicks away. So far, so good - but given all that is known about local seo (which is a very different beast than national seo) quite a bit of on-page/architecture work can still be done to further improve our local rank. So here a a couple big questions facing us at present: First, regarding format, is it a given that using geo keywords within the url indispustibly and dramatically impacts a site's local rank for the better (e.g. the #2 result for "tummy tuck" and its SHENANIGANS level use of "NYC", "Manhattan", "newyorkcity" etc.)? Assuming that it is, would we be better off updating our cosmetic procedure landing page urls to "/albany-tummy-tuck" or "/albany-ny-tummy-tuck" or "/tummy-tuck-albany" etc.? Second, regarding structure, would we be better off locating every procedure page within the root directory (re: "/rhinoplasty-albany-ny/") or within each procedure's proper parent category (re: "/facial-rejuvenation/rhinoplasty-albany-ny/")? From what I've read within the SEOmoz Q&A, adding that parent category (e.g. "/breast-enhancement/breast-lift") is better than having every link in the root (i.e. completely flat). Third, how long before google updates their algorithm so that geo-optimized urls like http://www.kolkermd.com/newyorkplasticsurgeon/tummytucknewyorkcity.htm don't beat other sites who do not optimize so aggressively or local? Fourth, assuming that each cosmetic procedure page will eventually have strong link profiles (via diligent, long term link building efforts), is it possible that geo-targeted urls will negatively impact our ability to rank for regional or less geo-specific searches? Thanks!
