Inurl: search shows results without keyword in URL

While doing some research on the indexation status of a client I ran into something unexpected. I have my hypothesis on what might be happing, but would like a second opinion on this.
The query 'site:example.org inurl:index.php' returns about 18.000 results. However, when I hover my mouse of these results, no index.php shows up in the URL. So, Google seems to think these (then duplicate content) URLs still exist, but a 301 has changed the actual goal URL? A similar things happens for inurl:page. In fact, all the 'index.php' and 'page' parameters were removed over a year back, so there in fact shouldn't be any of those left in the index by now. The dates next to the search results are 2005, 2008, etc. (i.e. far before 2013). These dates accurately reflect the times these forums topic were created.
Long story short: are these ~30.000 'phantom URLs' in the index out of total of ~100.000 indexed pages hurting the search rankings in some way? What do you suggest to get them out? Submitting a 100% coverage sitemap (just a few days back) doesn't seem to have any effect on these phantom results (yet).

We encountered something similar when we migrated a site. We properly redirected all the old url's to the new one, however, in the weeks after the migration, we saw a huge increase of 404 in the webmastertools.
When we took a closer look to these url's, we noticed that these where using an url structure we had abandoned several years ago. On the "old" site, these were redirected, but we didn't implement these old redirections after migration as we assumed that these very old url's wouldn't be in the index anymore. We proved wrong. We could delete them manually from the index using webmaster tools, because they used folders we are not using any longer, this is not probably not possible in your case.
While it is a bit annoying, I don't think that having these "phantom" url's in the index is doing you any harm in terms of SEO. They will probably never pop-up for normal search queries, only when you do in-depth queries, limiting the results to only your site.

A few days plus ~100,000 decade's old pages in Google is usuallynot enough time to see a change. You can spot check the 301s and run a fetch / render from GWT to see if the changes should be working though. Other than that, you'll probably have to wait a bit longer.

Google Search Console Showing 404 errors for product pages not in sitemap?
We have some products with url changes over the past several months. Google is showing these as having 404 errors even though they are not in sitemap (sitemap shows the correct NEW url). Is this expected? Will these errors eventually go away/stop being monitored by Google?

Another company is coming up in search results when I type in my phone number
If I search for my client's phone number on Google, without gaps, ie 02036315541, another company comes up at the top of the list. This company has a similar name to ours, but it is in a different town and it does different things. My company name is Energy Contract Renewals https://www.energycontractrenewals.co.uk/ and their company is https://energyrenewals.co.uk. As far as I can see, the other company does not mention our phone number anywhere on their site or on their GMB page so I don't know why they are coming up. We do not come up at all for this search. However, if I put our phone number in like this: 020 3631 5541, our company does come up and the other company does not. Anyone know how I can correct this or if it is even possible to do something about it?

Seeing URL Slugs as search result titles
I've been seeing some search results for my site that look like the first result here, where the URL slug is used as SERP title: https://drive.google.com/a/fitsmallbusiness.com/file/d/0B37y4RslpuY-a0hQYjlJQ0NxeFJicDF6RVlURFVSNFN0aGhB/view?usp=sharing The article title (and Yoast snippet title) are both "28 Press Release Examples From The Pros", but for some reason I'm seeing "press-release-examples" in the search results. I've seen this for multiple articles, and I see it now and then with different articles. I'm aware that Google often changes the titles in search results, but it seems very weird to me that they would opt for just the URL slug here. Thoughts? Has anyone else seen this issue? Any idea what might be causing this? All help much appreciated.

Url folder structure
I work for a travel site and we have pages for properties in destinations and am trying to decide how best to organize the URLs basically we have our main domain, resort pages and we'll also have articles about each resort so the URL structure will actually get longer:

Blog article URL - with or without date?
Quick question to all you folks: does including the date in a blog article's permalink affect rankings? For example, here's an article with the month and year, as well as the blog title: http://www.ayzanyc.com/blog/2012/12/difference-between-hot-chocolate-hot-cocoa/ Is it better to omit the date and just put the blog title? Also, if is better to avoid using the date, is it worth it to change the link structure of our previous articles (given that the URL will now be different), or should we just focus on future articles? Thanks ahead of time for your advice.

How to handle (internal) search result pages?
Hi Mozers, I'm not quite sure what the best way is to handle internal search pages. In this case it's for an ecommerce website with about 8.000+ products and search pages currently look like: example.com/search.php?search=QUERY+HERE. I'm leaning towards making them follow, noindex. Since pages like this can be easily abused for duplicate content and because I'd rather have the category pages ranked. How would you handle this?

How do I get Google to display categories instead of the URL in results?
I've seen that for some domains Google will show a nice clickable site heirarchy in place of the actual URL of a search result. See attached for an example. How do I go about achieving this type of results? categorized.png
