How are keywords assigned to the On Page Rankings

Hopefully this isn't a stupid question. I'm new to MOZ Pro so I created a campaign and uploaded about 300 keywords. Our site is an ecommerce site so there are many many products and of course, many pages.
My first On Page Ranking Report has completed and naturally we have pages with scores ranging from A to F. What I don't understand is this: for any given keyword, how did that word get assigned to a given page for the purpose of the ranking report?
We may have many pages that may include a given keyword so when I look at a page with a score of F, I'm sometimes surprised that a given keyword is assigned to a particular page for the purposes of ranking.
Can someone help me understand this architecture?

It's all good food for thought and I appreciate your taking the time to chime in. I've just begun working more hours at an eCommerce business and they rolled their own site so there is much to do in terms of SEO, duplicate content, duplicate title tags etc. Understanding how the MOZ reports do what they do helps me prioritize my work.

Most likely (and it will take a staffer on Moz to give correct answer) it is how it shows in the rankings. So, often when we create a page our intent is this is the page that will rank for "turnips." We do pages on carrots, celery, and potatoes, but we want our turnips page number one on a search for turnips. But...google doesn't always see it that way. I can laugh today about it but early on in my seo career it drove me to distraction. How in the __ could they think this is what I wanted to rank for?
Also, you can go on the page grader and put in the page you want to grade out for a given term. I understand the limitations given a large number of pages or terms.

Huge fluctuation reported by GSC for US Desktop for one keyword
Hi, I'm seeing a huge rank fluctuation reported by GSC (all rank trackers reported stable rank) for one of our pages, one particular keyword, in one particular country (US), and only Desktop (not Mobile). Is anyone else encountering a similar case? More details: huge rank fluctuation reported by GSC in the US, stable in other countries (since Oct 7th) all rank tracking service reported stable top rank in US cities (as well as our own friend's testing across the country) only happen to 1 keyword and its close variation (other keywords that the page also ranks for are quite stable) only observed fluctuation for Desktop, while Mobile is stable in those low-ranked days, the average CTR is only slightly lower than normal days Does anyone have any idea what could this be? Subdomain not showing up in ranking reports?
Hi Folks, Apologies for the newbie question here, but hope any of the experts here can help. Our blog resides on a subdomain, and our blog increasingly plays an important role in driving traffic to our site. However the subdomain does not seem to show up in our keyword rankings report on Moz pro. Does this report only include URLs on the main site? Is there a way to monitor how our blog ranks on our keywords somewhere else? Thanks in advance all! Regards Patrick

Getting errors -- Pages with Title Missing or Empty
I am getting a high priority warning that I have pages with title missing or empty, however, when I check the page, there is a title tag <title>Long Term Care Planning</title> . Anyone have any idea what could be doing on? I check the CMS (using AEM - adobe experience manager CQ5) and the title field is filled out as well.

Duplicate Pages...where are both URLs?
Just ran my Site Crawl Report...which was awesome! But It says there are over 50 Duplicate Content Pages, and the list of links only shows the PAGE that is supposedly "duplicate content"...but where is the other URL that these pages are duplicates of? Would be nice to be able to double-check if it is in fact duplicate and decide which to edit. Pls help! Thanks Moz!
