Submitted URL has crawl issue - Submitted URL seems to be a Soft 404 - but all looks fine

Google Search Console is showing some pages up as "Submitted URL has crawl issue" but they look fine to me. I have set them as fixed but after a month they were finally re-crawled and google states the issue persists.
Examples are:
https://www.rscpp.co.uk/counselling/175809/psychology-alcester-lanes-end.html
https://www.rscpp.co.uk/browse/location-index/889/index-of-therapy-in-hanger-lane.html
https://www.rscpp.co.uk/counselling/274646/psychology-waltham-forest-sexual-problems.htmlThere's also some "Submitted URL seems to be a Soft 404":
https://www.rscpp.co.uk/counselling/112585/counselling-moseley-depression.html
I also have more which are "pending", but again I couldn't see a problem with them in the first place.
I'm at a bit of a loss as to what to do next.

Hey Tommy! Are you able to post a screen shot of what you are seeing in the search console. Is your web host reliable to where your site is going down periodically? Just curious. I look forward to seeing the screen shot.

WP URL issue - Concatenated URLs (LOTS of them)
WP is doing this somehow, and creating URLs for hundreds of pages that don't exist. HOW is this happening, and how do I stop It? I have many, many URLS like this: https://www.atouchofrust.com/terms-of-use/atouchofrust.com/vendor-news. Of note, atouchofrust.com/terms-of-use, and atouchofrust.com/vendor-news are both legit pages on the site. Why they are being concatenated is beyond my limited understanding of WP. Special characters in URL
Will registered trademark symbol within a URL be bad? I know some special characters are unsafe (#, >, etc.) but can not find anything that mentions registered trademark. Thanks!

Noindex nofollow issue
Hi, For some reason 2 pages on my website, time to time get noindex nofollow tags they disappear from search engine, i have to log in my thesis wp theme and uncheck box for "noindex" "nofollow" and them update, in couple days my website is back up. here is screen shot http://screencast.com/t/A6V6tIr2Cb6 Is that something in thesis theme that cause the problem? even though i unchecked the box and updated but its still stays checked http://screencast.com/t/TnjDcYfsH4sq appreciated for your help!

I have altered a url as it was too long. Do I need to do a 301 redirect for the old url?
Crawl diagnostics has shown a url that is too long on one of our sites. I have altered it to make it shorter. Do I now need to do a 301 redirect from the old url? I have altered a url previously and the old url now goes to the home page - can't understand why. Anyone know what is best practice here? Thanks

Website Ranking Issue
Hi, We have been performing our own onsite of offsite SEO along with external assistance and have ranked well over the years with minimal impact from Google updates. Howevr the last so called Panda update has affected us heavily pushing our main phrase 'web design melbourne' from 2nd to 7th where we have been for almost 2 months now on Google.com.au irrespective of onsite or offsite work. We have been trying to find signs of any onsite, IP, duplicate content, titles or other issues that may be holding us back to no avail. The only flag that Google webmaster tools is showing is a number of bad internal site links, which I think is a glitch with the CMS we are using. Even the SEO MOZ tool gives us a higher ranking compared to most competitors on page 1 of Google.com.au for our main phrase. The biggest difference between us and competitors is we chose to target an internal page specific to the topic rather than our homepage. With this sadi we have also reduced our keyword density and content quantity inline with the other sites homepages. Can anyone help shed some light on this? and perhaps something obvious that we have missed, or where we should be looking? Thanks.
