On-Page Grader giving wrong results for missing keyword in image Alt Attribute

When I try to run a Moz report, it sends me to a 404 page?
Hey there. I'm trying to export a .pdf to send to my client. When I click "export pdf", the page sits for a second then goes to a 404 page? I've never seen this before. Is anyone else getting this problem?

Moz Top Pages and Tool Bar Not Crawling Internal Pages and Links
Hello, We’re having two issues with our Moz tools and we’re not sure what’s causing them and whether they are related. The Moz Bar isn’t highlighting some of our internal links (including navigation links). The Top Pages Report in Open Site Explorer is only picking up the homepage and a couple error pages (none of the internal pages). The full Crawl Report is picking up everything though. Could a potential cause of both these issues be the Title attribute in some our links? – We use <a <="" span="">title="Example" href="link"></a> <a <="" span="">Or is this most likely from something else blocking the crawler from accessing our links/pages? Google Search Console does seem to be picking up the links in the navigation and everything is indexed/rendered correctly so we also didn’t know if this is something that could be issue. We have proper markup schema installed in webpage (validated using data structure tool by google) but moz tool bar says "Schema.org not found on this page." What should be the problem ?

Cannot Crawl ... 612 : Page banned by error response for robots.txt.
I tried to crawl www.cartronix.com and I get this error: 612 : Page banned by error response for robots.txt. I have a robots.txt file and it does not appear to be blocking anything www.cartronix.com/robots.txt Also, Search Console is showing "allowed" in the robots.txt test... I've crawled many of our other sites that are similarly set up without issue. What could the problem be?

Should I be getting an 'A' Grade for basic words in a page title?
It seems as long as a word in the page title matches a word(s) within the page content you will get an 'A' grade. Should I be replacing these keywords with higher monthly searches and lower difficulty? It appears deceptive when you test a new website and all the pages get an 'A' for a word as basic as 'about.' Please advise.
