what does moz consider to be an "indexed page"? is it a number of words? paragraphs?

A page that is in the Search engine index.do a search for site:mydomain.com and you will see your indexed pages

An indexed page is simply any page that has been crawled and then pulled into the search engine's index of web pages (Google's, in most contexts). Until a page is in the index, it can't be returned in the SERPs for any search query. Pages that haven't yet been discovered by Google, or that are somehow blocked for indexing (such as by a noindex directive or a robots.txt file blocking the bots), won't show up in the index. In the Google search box, you can enter "site:yoursiteurl" (without the quotes) to see the pages that are in the index.

Cant figture out why On-Page Grader is not recognizing the heading tag text
Hi all, I'm using the On-Page Grader tool to optimize some of our pages: https://analytics.moz.com/pro/research/page-grader?keyword=Russian Immigration Lawyers in Sacramento&url=https%3A%2F%2Fwww.russianlawyers.us%2Fus%2Fcalifornia%2Fsacramento-immigration One of the suggestions is "Optimal Use of Keywords in Header Tags". They keyword is "Russian Immigration Lawyers in Sacramento" and we do have it in an tag on the page (located in the right sidebar). The tool doesnt seem to catch that though. Can someone help me please? Thanks

How can a page have high Google/ organic traffic but show no ranking keywords in Moz?
We have a page on our website with a higher than average number of pageviews, 85% of which came from Google organic search. When I research this page by entering the URL into the "exact page" keyword research tool, Moz says it has no ranking keywords. How can a page be earning organic traffic without ranking for any keywords?

On Page Grader - URL not accessible
We have tried to use the On Page Grader today and it is coming back with URL not accessible for all pages on our website. We previously used the On Page Grader on Friday 10th Nov for a couple of product pages with no issues. Since then, the only changes we have made on the websites is updating some downloadable documents. We have done this several times before and it has never affected Moz. We have not changed the page URLs, and therefore do not know why it is now not working. The pages are working fine on the website with no issues. On Page Grader inconsistent
Why does the on page grader not update it's grades based on the other factors, other then the title tag. i.e. I can have the keyword 'burger' in H1 tags, the URL, ALT attributes, in the body text a couple times and in the meta section of the site and receive an F grade from the on page grader, but as soon as I add the word 'burger' to the title I receive an A grade. Is there a reason why the only factor that has that influence is the title tag, and why is it that a keyword for a page cant get say a B or C grade if it has the other factors covered (i.e. they have a tick next to them) but not the title tag? Rank Tracker page shows only first 50 entries
On the Rank Tracker tool page, http://moz.com/researchtools/rank-tracker, when I hit the Next 25 it works the first time but none of the others. Trying to run a report and since we have more than 50 keywords I have to manually click on them to be checked.
