I presume this is all wrapped in an Organization element, or something like that?

The itemprop="address" in that first div says that this next block describes the address of the element that's enclosing all of what you have here.  I.e. if you're using schema.org/Organization as your outer element, that itemprop="address" says that the div there describes your company's address.  Within that div, then, you have all the components of the address.

It doesn't hurt to have the telephone at both levels.  Ditto email.
