Authorship Not Displaying

I've been reading through a ton of articles on Google Authorship on Moz as well as a variety of other sites but still haven't solved my issue.
I set up authorship a while ago and it stopped working after a very short time. It came back and again stopped working. I use Yoast for the tags.
I realize that Google drastically decreased showing authorship photos in SERP but I've never been about to get it to show no matter what I've tried aside from that brief moment of success when I originally implemented the tags.
Rich snippet tools shows all as being a ok!
Any suggestions what I might be doing wrong?

Hi there,
The authorship is based on the personal page not the business one. I suggest you get some followers (or groups) on your personal page and use that a bit. Business page is more of a rel=publisher tag.

Thanks for the reply! I have 2796 followers on my g+ website page....I typically post my latest posts on that page. However, I'm not terribly active on my personal g+ page.
Just regular personal posting on my personal g+ page will help?

Would agree With Dennis,
Make sure its working alright with the testing tool other than than that there is not much you can do. Make sure you are active on Google plus with a following and you may get it back but the authorship was decreased as you noted.

Are you active on google+?
If you are, with a good following, you should get your image back.
They will email you that you are now showing up as an author or something like that.
Although this doesn't apply to everyone, this has been what has worked for me.

Google displays multiple titles for same article. What does this mean?
I've linked to some screenshots so that it what I'm talking about makes more sense. Sometimes, when I perform a search, I see an article with the correct article title listed as the page title in the SERPs. Other times, I see the wrong page title – it's a generic somethin' or other done by my client's web design company with a bunch of keywords thrown in. The latter (not the correct article title) also appears at the top of the browser tab for every article on my client's site. I know this is bad, but what can be done about it? Google authorship syntax, plus no follow
I have seen two forms of rel=author syntax. Are they both valid? (1) (2) Second, does 'no follow' take away authorship? Is there any point in doing rel=author for a link that is rel=no follow? Like this:

Authorship and Publisher on WordPress
I successfully enabled rel=publisher on our WordPress blog, and as a test I also enabled rel=authorship for a set of blog posts. (Tested both in Google's Rich Snippets Tester.) However, on the individual blog posts the publisher credit disappears. Is there a way to enable both to appear on blog posts?

Why is Google not displaying the right URL on SERP?
Google is not displaying the URL correctly for this page. Displaying static content - risky?
In an attempt to improve the speed of our site, We have installed Cache_Lite extension for PHP. Its a PEAR based system which converts dynamic pages into static pages. The system is set to delete the temp files every 15 minutes, at which point any changes / new content will appear on the system. I don't see any risk of doing this but thought it safe to double check if there could be any impact on Google by doing it?

Display: none
A prominent SEO consultant recently told me that using display:none in the css is still a bad idea. We built a simple drop down menu using widely used jquery techniques and it uses display:none to slideToggle the menu. Does anyone else think this is a bad idea? If so, how should I change it since just about every jquery drop down I come across uses the slideToggle/display:none.
