Using hreflang tags properly.

On my site "example.com" I have set up the following in the header:
The problem is that the tags are universal across the site, so every page has these tags, leading obviously to no return tag errors. I.e. the page www.example.ca/testing.html still has the tags:
Not tags with "testing.html" in them. How bad is this? Does it matter?

very bad. you are better off not using the hreflang tag at all then... you'll just be confusing search engines.
www.example.ca/testing.html should have:

Google not detecting Hreflang
Hey everybody, We recently migrated our .co.uk to .com/en. Google for some reason is saying that the .com/en version has no hfrelang tags - even though they are clearly there and have had the same implementation as other language versions of the website. We also did a previous migration 6 months ago for the german version of our website and no hreflang problems there. We add our hreflang tags to our sitemap - which you can find here:

General questions about implementing hreflang using XML sitemap
I created another thread regarding hreflang sitemaps. However, this one is more general and doesn't cover multiple sitemaps for different localizations so I think it's reasonable creating a new thread. We are trying to implement hreflang using XML sitemap. We have localized content for a few countries, but only 1/3 of the content is 'duplicate' localized content. How should this be presented in the sitemap? Can we have some urls with hreflang-tags and some without? Also, where should this be located? In the usual sitemap file at site.com/sitemap.xml or should we create a different sitemap site.com/hreflang.xml where we just paste all hreflang-info? And if it should be in /hreflang.xml - can we have the same URL twice (in both current sitemap and hreflang sitemap)?

Google changing title tags to branded
Hi guys and gals, i noticed in the results today Google has been changing my title tags so that brand: is in the front of it all. This is not in the code but my title tag structure site-wide is keywords and then always | brand on the end. Why has this happened does anyone know? does it affect my first keyword in the title tag, i guess Google might do it as it recognises the brand and wants users to know it?

Canoical tags how do i use them
Hi i have this coming up on the report for my url www.in2town.co.uk but i am not sure how to use the canonical tag. I am using joomla and would be grateful if anyone could please give me advice on how to use this. According to SEOMoz platform, one of my wordpress websites deals with duplicate content because of the tags I use. How should I fix it? Is it loyal to remove tag links from the post pages?

Do I need to add canonical link tags to pages that I promote & track w/ UTM tags?
New to SEOmoz, loving it so far. I promote content on my site a lot and am diligent about using UTM tags to track conversions & attribute data properly. I was reading earlier about the use of link rel=canonical in the case of duplicate page content and can't find a conclusive answer whether or not I need to add the canonical tag to these pages. Do I need the canonical tag in this case? If so, can the canonical tag live in the HEAD section of the original / base page itself as well as any other URLs that call that content (that have UTM tags, etc)? Thank you.

Is using a Href in Div OK?

Duplicate content and tags
Hi, I have a blog on posterous that I'm trying to rank. SEOMoz tells me that I have duplicate content pretty much everywhere (4 articles written, 6 errors at the last crawl). The problem is that I tag my posts, and apparently SEOMoz thinks that it's duplicate content only because I don't have so many posts, so pages end up being very very similar. What can I do in these situations ?
