Missing Title tags

My crawl diagnostics shows roughly 20+ pages with missing or empty title tags, however, the titles are viewable through the source code and there is only one instance of it on the page. This is not a wordpress site nor are the titles extremely long. Would there be any other reason for the crawl tool to not pick up these titles?

I get this explanation on the grader: Although using targeted keywords in H1 tags on your page does not directly correlate to high rankings, it does appear to provide some slight value. It's also considered a best practice for accessibility and helps potential visitors determine your page's content, so we recommend it. Over-using keywords, however, can be perceived as keyword stuffing (a form of search engine spam) and can negatively impact rankings, so use keywords in H1 tags two or fewer times. To adhere to best practices in Google News and Bing News, headlines should contain the relevant keyword target and be treated with the same importance as title tags. But I have the keyword in an H1 tag, only once. So why am getting this message on the grader? Only One Canonical URL Tag
HI, I'm an SEO novice - company owner with no money so doing it all myself with help from my web designer using wordpress. Ive just completed some seo and done the moz page scoring analysis for optimisation and gained 92% - however - there is one outstanding issue on canonical url tags - i.e. recommened fix = The canonical URL tag is intended to refer duplicate pages to a single canonical URL. To ensure the search engines properly parse the canonical source, your page should use only one version of this tag in the header. See Canonical URL Tag - the Most Important Advancement in SEO Practices Since Sitemaps Ive gone through the page code and can see I have 2 rel=canonical references - am I able to simply delete one - how do I do this if its been created by the yoast/wordpress plug-in? How to find a list of pages with missing H1 tags
An external SEO/PPC agency did an audit of our site a little while back and said that over 10% of our pages were missing an H1 tag. I am trying to find a way to gather a full list of these in order for our web company to fix. I downloaded the Crawl Test report thinking it would include info on tags in there but it doesn't seem to. Is there a different tool I can use that will get me this information?

The website is www.bigbluem.com and is a wordpress site. I'm getting the following error: 605 Page Banned by robots.txt, X-Robots-Tag HTTP Header, or Meta Robots Tag But what is weird is the domain it lists below that is http://None/BigBlueM.com Any advice?

Should I be getting an 'A' Grade for basic words in a page title?
It seems as long as a word in the page title matches a word(s) within the page content you will get an 'A' grade. Should I be replacing these keywords with higher monthly searches and lower difficulty? It appears deceptive when you test a new website and all the pages get an 'A' for a word as basic as 'about.' Please advise.

OnPage Reports - Duplicate titles and meta descriptions

On the old interface, I used to use a report that would automatically run when I created a new account letting me know where the dup titles and meta descriptions were on an entire site. Clarify "broad keyword usage in page title"
Hello Page grader has two different grades for page title that I want clarification on. There is "Broad Keyword Usage in Page Title" and "Exact Keyword Usage in Page Title". Googling around about and searching here I have found that "broad" seems to mean the keywords should be used throughout the page, rather than just in the title and header. Which makes sense as this is a kind of check to ensure the page IS about the keywords and not something unrelated. But what is meant by "broad" usage in the page title? This refers specifically to the page title and not the whole document. My best guess got me to this, given the keyword "Visit London Today"; "Come and visit London today" - exact match only "London - visit today" - broad match only "Visit London the city of dreams | visit London today" - matches both That could be complete nonsense, but basically is broad usage the use of keywords scattered in the page title? Thanks.
