Bad on page optimization grade, still displays high in SERP

I'm looking at the keyword on page optimization report for my site and I have keywords where my landing page is showing up first on the SERP, but my SEOmoz on-page optimization grade for them is an F. Am I ranking high for these terms because no one is optimized for the terms? They are almost all directed at my home page, does domain authority play a role in me ranking high for them? Thanks for any insight.

Hi, there are more factors that play into ranking then on-page optimization. In the 2011 ranking factors, you can see that Page Level Link Metrics and Domain Level Link Authority all play a larger role then on-page keyword usage.
To give you an example, Geek Squad ranks in the first few results for "computer repair" but they score an F for on-page optimization. (imagine our frustration)
Getting high quality links from powerful domains, and getting links with powerful anchor text can account for your situation. I am curious as to why your page isn't optimized for those specific keywords? Did you do this intentionally?

Keyword stuffing - on page grader count
I used the on page grader for my homepage, www.cprnj.com, and it said the keyword "physical therapy" was used 26 times in the body of the page. But I can't find more than 4. How exactly is keyword stuffing accounted for, and is there some place that I am not seeing the keywords?

Getting a URL Unaccessible on the page grader
I'm optimizing a site for a financial advisor, here is the site: http://www.mattkeenancfp.com I am getting the message "that URL is unaccessible" when I try to use the on-page grader. This is an emerald website too, I'm not sure if that has any effect on anything though.

Rel="canonical" tag is implemented in my product pages, but still getting canoncal error for products in Moz. What is the problem? me or MOZ?
I have included the rel="canonical" tag in all my product pages, but still getting canonical error in MOZ reports for more than 6 month ! I would like to know if my code is wrong or the MOZ report system is not working properly. Duplicate page content on / and index.php
Hi I am new to SEOmoz and in the crawl diagnostics for one of my clients it came back duplicate content on the homepage www.myclient.co.uk and on the www.myclient.co.uk/index.php which is obviously the same page. I understand that the key is to do a 301 redirect from the index to /, however how will I know that this will not just create an ever ending loop on the server? From your experience how is the best way to tackle this crawl error? Also is there a specific question that I need to ask the server?

Hi Seomoz - first of all, thanks for the best SEO tools I have ever worked with (this is my first question in this forum, and also I just subscribed as a paying customer after the 30 days trial you guys offer). My question: After having worked for several weeks on getting the numbers of links in our forum on www.texaspoker.dk down, we are somewhat surprised to see that we didn't succeed in getting lower numbers. For instance, this page: http://www.texaspoker.dk/forum/aktuelle-konkurrencer/coaching-projekt-bliver-du-den-udvalgte has (that's what Seomoz seo tool tells us): 239 on page links. Can this really be true? We can't find these links, and we actuually did a lot to lower the numbers of links, for instance the forum members picture was a link before, and also there was a "go to top" link in each post in the forum. How does moztrust compare with google page rank?
I have a trial Seomoz membership. I've been comparing my site to some of the competitor websites and Im confused about the moztrust vs google page rank. For instance some of my competitors have higher moztrust but my site has a higher google page rank. Isn't moztrust meant to duplicate google pagerank?? Why would these vary so much?
