SEO for Explicit / Medical Pictures - Will It Hurt Rankings?

I have a client with page 1 rankings for several keywords. He wants to make some updates to his website.
He is offering a new service and wants to show before and after photos of the procedure. He is an OB/GYN, so the before/after pictures are very explicit and have full nudity.
How will this affect his rankings? Is there something that I can do to tell Google and other SE's that these photos are medical and not pornographic? Should I host them on another domain? Just looking for thoughts and insight. Thank you in advance!

Thanks for your response. A few weeks ago we actually posted the pictures right on the page. This page is indexed / followed- everything. Google hasn't penalized the site or anything like that. If anything, it is helping his visitors by showing his work. So I guess until Google can "see" the image, keeping them on the page will be the best option. Thanks!

This is off the cuff - but may help your approach to this. (I too have a medical client)
We know Google can't see the image (yet!!)
It can read the jpg file name and alt tag
Two ideas:
If it is a new service why not password protect the area with sensitive photos
or
Create a link which takes them to a gallery. Explain the link is to a gallery with photos that may be distressing.
that they click agree to continue and could always have a data capture to prevent children finding it
You could then have the gallery pages with a no follow robot so the page is not cached and they aren't found through any search, and that way you can still optimise the rest of the site / or hold onto your existing rankings
Further to this if there were any 'before' and 'after' photos that could be displayed - they could be done in page and the link for more case studies underneath.
Also
If you were concerned about any language you may have to use embed it in a graphic button.
Just a few thoughts.

CDN image links passing SEO benefit?
We used a CDN to host all of the images on our site, and the paths are such:

Ranking well for keywords, but not in Google maps
If the search "criminal defense attorney" or any number of variants, is performed from a mobile device, like an iPhone, Google maps seems to be inserting many local map listings ahead of the normal search results in the Tallahassee, Florida area. These are great - as they have the Call button and the Directions button. However, for some reason, my clients aren't ranking well here and competitors who are seemingly doing nothing are higher in the list. I have created the Google+ page for attorney and business page for the firm and completed the profile, uploading pictures etc and verified the address. I have the local address (schema-proper, I believe) on the linking home page and their address is actually closer to downtown/midtown than others. I've even gotten some positive reviews and +1s, at least enough to be competitive in this field. This was about 2 months ago. Any advice? On a (perhaps) related note, the competition has category descriptions outside of the Google-allowed categories. I can't save settings without fitting neatly into their "Criminal Justice Attorney" category. Help here would also be appreciated.

Image SEO in 2013
3.26.13 Everything I can find on image SEO is relatively dated and some that I do don't explain exactly what should be done. 1. When designing a site it is easier to simply integrate best practices from the get go. So what are the best practices? Alt tags - these are to explain the photo for handicapped individuals but you can naturally weave in a good keyword. Image titles - do these matter? URL of the jpg - should we put in a good keyword here? What about schema? What about Meta tags for the page the pic loads on? What about pinning to Pinterest and what is properly pulled over? What about other social channels and sharing? What am I missing? Who out there has the ultimate guide for optimizing images in 2013? Thank you!

What's the deal with this local business directory? (http://www.camerchants.com/)
They have a couple listings of us with screwy info but i can't actually use their website, all the buttons that say "join" or "write a review" "uplaod an image" etc, are just an href="#" and don't actually do anything??? I figured maybe they have some broken javascript so i tried using different browsers (firefox & chrome in mac, and IE in windows.)
