Alt attributes same as jpg name and keywords?

Hi everyone,
Quick question: Is it better to have your jpg name and alt text slightly different to your keywords for that particular page, or is it better to have them slightly differently? At the minute I'm doing them all with a variation on the keyphrase I want to optimize for (long tail and all that...).
Any input much appreciated!

Since the intention of alt text is to help the user understand what the image is about, it should use natural explanations which utilize a range of variations on your keyphrase(s).
Keep it natural and keep the user in mind.

Hello, if I am trying to rank for a service type and the exact phrase is in the companies name, can I put the company name in the image alt text instead of just the service type? Seizing the opportunity to get another mention of the businesses name...two birds with the one stone if you will... For example, "lawn cutting" being the service type but the company's name is Paul's lawn cutting. Could i put Pauls Lawn Cutting in the image alt text, or just stick to lawn cutting? Chasing a particular keyword (and failing)
Hello, Its probably the most common and annoying questions, but I'm pulling my hair out a bit (not helped by not really knowing what I'm doing). I'm trying to optimise this http://www.architecturaldecor.co.uk/collections/antique-door-handles collection page for the main keyword 'antique door handles' secondary keyword 'antique door knobs'. Mid February I changed the URL, title and description and it started to move up the rankings, I then tidied up the descriptions, titles, ALT tags for all the products in that collection and it moved up to #13 quite quickly. However over the last 10 days it dropped down page 2 and today it's down to page 3. Whats more frustrating is 90% of those ranked in front of me don't actually sell antique door handles. When I tidied up the products in that collection I was increasing using the term 'antique door handles'. Any suggestions would be really, really welcome, have I overdone 'antique door handles' in the product descriptions. Thanks in advance, Paul.

Are Meta-keywords coming back?
I'm currently doing some benchmarking for a big realtor site here in México, while looking at the biggest players in the US I noticed most if not all are using the Keywords meta tag in their detail listings. I've been really open to my client about not using this tag at all given the current common knowledge but when sites like: Trulia.com, Realtor.com, appartments.com and the like, are using them I'm second guessing their utility. Does anyone have any insights on this? Should or should not we use meta-keywords? On a side note, there is some interesting microdata going on, in those sites.

Image file name, is it important
If I use the same image all over my site, do I need to change the file name to avoid duplicate? Different alt text will be use on those images
