Creating sitemaps

What I'm using with great success is XLM sitemap generator (xml-sitemaps.com).
It is $19, I think and you'll have to upload it in a separate folder.
You can highly configure it and specify a wide range of parameters (to exclude or include in the sitemap). You can also setup a cron job so you wont have to run it every other day.
I am not affiliated in any way with them, I am just saying that it might be exactly what you need.

A little more info would be useful. How is your site structured? Do you use a CMS?
I use Yoast's SEO plugin for wordpress which is excellent. You can define which categories or post types you want to be in your sitemap and which you want to be left out.
Maybe if you are more specific with your question someone can help with a more specific answer

Should all pagination pages be included in sitemaps
How important is it for a sitemap to include all individual urls for the paginated content. Assuming the rel next and prev tags are set up would it be ok to just have the page 1 in the sitemap ?

How can I best find out which URLs from large sitemaps aren't indexed?
I have about a dozen sitemaps with a total of just over 300,000 urls in them. These have been carefully created to only select the content that I feel is above a certain threshold. However, Google says they have only indexed 230,000 of these urls. Now I'm wondering, how can I best go about working out which URLs they haven't indexed? No errors are showing in WMT related to these pages. I can obviously manually start hitting it, but surely there's a better way?

3 URLS Being Created All For The Same Page
I use wordpress for my blog and for some reason it is creating triple urls for my pages. I am not sure it has always been like this or not. I just noticed it in the errors section of SEO Moz. http://www.kisswedding.com/blog/?gid=7&r=20 http://www.kisswedding.com/blog/ashley-and-daniels-rainy-day-diy-farm-wedding/?gid=7&r=20 http://www.kisswedding.com/blog/ashley-and-daniels-rainy-day-diy-farm-wedding/ It's all the exact same page. Is there something I can do in my settings to make this stop. I don't imagine this is good. Ya think....ha! I saw this is the SEO Moz error area for Missing Title Tags. Apparently the number has gone from 200 to 400 which is weird because I never gave my blog posts meta stuff and I haven't written 200 pages since SEO Moz's last crawl.

No inbound links. Should I link-build or create new content?
I have a PR4 site with good traffic but the blog is not very popular--the posts do not generate any backlinks and hardly get any traffic. Yet, I continue to kick out a new post every week. Site: http://www.stadriemblems.com/

Google Webmaster Tools reports 404s for plain text. Should I create those URLs and 301 them to actual pages?
IA few years back I noticed that Google Webmaster Tools returns 404s from regular text containing a URL, but no anchor tag. I came accross this again today. Is it worthwhile to create those URLs and 301 redirect them to proper pages.

Sitemap question
My sitemap includes www.example.com and www.example.com/index.html, they are both the same page, will this have any negative effects, or can I remove the www.example.com/index.html?
