Local Keywords Not Ranking Well in a Geographic Location (but Rank Very Well Outside of Geographic Location)

Has anyone experienced, in the last few months, an issue where a website that once ranked well for 'local' terms in Google stopped ranking well for those terms (but saw a ranking decrease only within the geographic location contained within those keywords)?
For example only, some 'root' keywords could be:
- Chicago dentist
- Chicago dentists
- dentist Chicago
- dentists Chicago
What happens is that when a searcher searches from within the geographic area of Chicago, IL, the target website no longer ranks on the 1st page for these types of keyword phrases, but they used to rank in the top 3 perhaps.
However, if someone was to search for the same keyword phrases from another city outside of Chicago or set a custom location (such as Illinois or even Milwaukee, WI perhaps) in their Google search, the target website appears to have normal (high) 1st page rankings for these types of terms.
My own theory:
At first I thought it was a Penguin related issue but the client's rankings overall haven't appeared to have been affected on the date(s) of Penguin updates. Authority Labs and Raven Tools (which uses Authority Labs data) did not detect any ranking decrease and still reports all the local keyword rankings as high on the 1st page of Google.
However, when the client themselves goes to check their own rankings (as they are within that affected geographic area), they are no where to be found on the 1st page. :S
After some digging I found that (one of) the company's Google Places listings (the main office listing) became an 'unsupported' status in Google Maps. So now I am thinking that this phenomenon is due to the fact that other listings are now appearing in search results for the same location.
For example, in this case, an individual dentist's Google Places listing (who works within the dental office) is being displayed instead of the actual dental office's listing. Also, the dentist's name on the Google Places listing is being swapped out by Google with the name of the dental office, but if you click through to the Google Places listing, it shows the name of the individual Dentist.
Anyone encounter a similar issue or have any other theories besides the Google Places issue?

Thanks Miriam, I'll take a look at those resources.
I actually have a correction to make in my original scenario. It is not that the place is 'unsupported', it is rather that it seems that one listing merged with another listing.
Thus the main office listing (along with 3 positive reviews) merged with one of the individual dentist listings.
It is worth mentioning that one Google rep did reply to me regarding another incident saying to check if the place was existent in Google Mapmaker. This opened up a whole other ballgame. If Google is using data from Map Maker, I can see why there are issues. That is a user driven database, verified by volunteer 'reviewers', etc...
I see some of the change histories for some of the places there a little scary. I think some 'users' don't realize that it is within Google's guidelines to have multiple listings for a single place if they are lawyers, dentists, etc.... or perhaps like some of the comments in the article links you posted mention... the listings are prone to competitor sabotage.

Rankings Dropped to Nothing
We're kind of in crisis mode, as our ad revenue is about to take a huge hit. Hoping someone can help me figure out what to do next. Site: https://indoorgardening.com Here's what I did (below) that I think broke things somehow. I'm clearly not an SEO expert but thought I was making things better. And things did improve over the last week or so but then fell apart 2 days ago. 1. Most posts did not have a Yoast focus keyword. I added keyword phrases and used Yoast suggestions to optimize for that and for readability. 2. In some cases I changed post titles to better reflect the keyword phrase. 3. In some cases I changed the slug per Yoast's suggestion and did a 301 redirect from the old slug to the new one. 4. I used Grammarly to fix all spelling, grammar, punctuation, etc. 5. Some Yoast-suggested changes that were made: Image alt tags, subheading structures, adding keyword to subheadings, first paragraph, and meta description, changed sentence length for those over 20 words to clean up the text, added transition words where applicable, reworded passive voice sentences, added internal links when needed, eliminated consecutive sentences (first word), improved Flesch reading ease when necessary. 6. I also added or changed Amazon affiliate links where needed and swapped out images when necessary. Results:

I started this project about 3 weeks ago. On 11/29 we had one of our highest traffic days, with 1017 hits coming from Google. On 11/30, 257 hits came from Google, and on 12/1, 3 (three!!) hits came from Google. At this point, 82 of 89 posts have a double green "Good" score in Yoast; 6 are "OK" and 1 does not have a focus keyword designated. Thanks in advance for any help anyone can provide. -John0 -
Pages Fighting Over Keywords
Hi Guys, Just after some general advice. Since manipulation of keywords through links is no longer a feasible way of ranking these days, I was wondering how people got round the issue of pages bouncing for the same keyword or Google deciding that a blog post is a better signal rather than your service page. For instance if you are doing local and national search, how do you stop the local keywords ranking for national pages, without diluting the local signals. I have some ideas:- stronger internal linking to the page review content But obviously redirects or canonical won't be a good solution as I still want these pages to exist in their own right. Keywords, when are you overdoing it in the URL?
Hi guys, I'm auditing a site covering compensation for cancer. Keywords could include: Undiagnosed cancer 20 cancer compensation 10 undiagnosed cancer symptoms 10 cancer misdiagnosis claims 20 cancer claims 10 misdiagnosis of cancer 50 cancer misdiagnosis 70 So, when structuring the URL for the category, this was previously selected: www.site.co.uk/medical-negligence/cancer-misdiagnosis Although sub-pages appear like this: www.site.co.uk/medical-negligence/cancer-misdiagnosis/breast-cancer-misdiagnosis-claim/ 'Cancer misdiagnosis' as a keyword attracts the most traffic, but if we're using it on sub-pages - is there a need to include it twice on all sub-page URLs? With that in mind, would it be better to follow the following format? www.site.co.uk/medical-negligence/cancer-compensation www.site.co.uk/medical-negligence/cancer-compensation/breast-cancer-misdiagnosis-claim/ Or is there a better way to structure this? How to target long tail keywords
Apologies if this is already answered, I'm a newbie. I'm looking to determine how to target long tail keywords. I'm supporting a site that is new and with very small budgets in the very competitive life insurance market. If we devise a list of long tail keywords, should we then add those to the page titles, meta descriptions etc rather than the short tail ones that the site will never rank for anyway?

My keyword ranking suddenly went down in google.
Hi, Need urgently help, one of our website http://www.bannerbuzz.com keywords ranking is down suddenly since 6-11-2012, even we don't have done any changes on the website & we were on top on many keywords. We have done only one change recently is we have moved our website from shared server to dedicated server on last 21st October & we can see some traffic fluctuation after to redirecting from shared server to dedicated server, also website load time has been increase in Google Analytic, so it that reason to get rank down suddenly?, because all keywords has been down. We have also some affiliate sites like bannerbuzz.ca, bannerbuzz.co.uk & bannerbuzz.com.au which design and coding structure is as same as bannerbuzz.com, so is that a problem (Duplication issue)? We have tried to find out the solution for the same, but we didn't get any proper reason to get rank down suddenly. Can it happened due to dedicated server which we have moved recently or Google has changed anything in algorithm? Can we put website in Google reconsideration to know proper reason about the website? Setting a geographic target in webmaster tools
If a site is targeting traffic from around the world should I set the geographic targeting in webmaster tools under 'settings' or leave it? Any help would be much appreciated!

Sudden SERP stop for main keyword.
Hey guys, I have a website with about 30 Google indexed pages. Most of my pages are perfoming well for my keywords. My homepage recently started doing extremely well. Number 4 overall for a highly competitive keyword/niche. Then after a week it dropped completely off the map. It still shows for secondary and tertiary keywords. But my primary keyword does not rank anywhere. I checked as deep as 50 pages. It's been 3 weeks now. Any ideas of what might have happened? Is there really a sandbox? Could I maybe have been Panda slapped? Or do you think it is more likely just a technical/SEO issue. It just seems odd that it jumped so high and then disappeared??
