Why isn't a 301 redirect removing old style URLs from Google's index?

I have two questions:1 - We changed the URL structure of our site. Old URLs were in the format of kiwiforsale.com/used_fruit/yummy_kiwi. These URLs are 301 redirected to kiwiforsale.com/used-fruit/yummy-kiwi. We are getting duplicate content errors in Google Webmaster Tools. Why isn't the 301 redirect removing the old style URL out of Google's index?2 - I tried to remove the old style URL at https://www.google.com/webmasters/tools/removals, however I got the message that "We think the image or web page you're trying to remove hasn't been removed by the site owner. Before Google can remove it from our search results, the site owner needs to take down or update the content."Why are we getting this message? Doesn't the 301 redirect alert Google that the old style URL is toast and it's gone?

Point 1 - it takes some time before Google takes redirects into account - if you changed the structure recently, it can take several weeks before these duplicates are removed from Webmastertools. I assume you have already checked that these url's are indeed 301 redirected by testing them with web-sniffer.net and/or Fetch like Google.
Point 2 - you can only remove content in the following cases (source: https://support.google.com/webmasters/answer/1663419?rd=1
- content is removed and generates a 404 - as you redirect the old style url to the new one it's generating a 301 & not 404 so removal is not possible this way
- the content that needs to be removed is blocked in the robots.txt - which I guess is not the case for you

It is possible that you still have it in the sitemaps. Did you check that?

Silo architecture and PR dilution! What's real?
Hi all, Today I have gone through this "Silo" concept where we need to build 2nd hierarchy level pages and then lower hierarchy pages further to rank good for related terms of "keyword(s)". But I wonder, is it real? the so called Silo structure? Google may consider that we are trying trick if we create multiple pages (doorway pages) targeting same keyword. And one of my competitors is having too many 2nd hierarchy level pages against this Silo structure and even the homepage rank may dilute by contributing to the so many pages. But their web pages rank good for the keywords they chosen by creating multiple landing pages. These are contrary to each other. How it works in real? Thanks

Include placename in URL, or not?
Hi Mozzers, I'm wondering whether to put placename in URL or not. This is for a hotel so it's very focused on the county. I have loads of sub pages along the lines of www.hotelname.com/short-breaks-somerset www.hotelname.com/eat-out-somerset and so on but I was wondering whether that placename element would help or hinder. For example, may want to rank for short breaks in other searches (not just those seeking short breaks in Somerset) and was wondering whether the somerset bit may actually hinder this in the future. Also noticed Somerset is mentioned in nearly all of the page urls through the site. Perhaps this is a bit spammy and just not neccesary. I can include the address of the hotel on every page anyway. Has my site been penalized by google
Hi all I have noticed a sudden drop in rankings for most of my keywords on kerryblu ,co,uk and was thinking the site may have been manually penalized by google. I have not received any notification of this in webmaster tools but can't think of any other reason for the loss of rankings. I have searched the web for info on this but can't find a definite answer. Is there any way of knowing for sure. At the time of the crash the only real change I made was adding google adsense to my blog. Could this be responsible. Thanks for looking.

Is Google now punishing anchor text?
Hi All, I was just wondering if Google is starting to punish anchor text links? I've noticed that one of my clients domains has slightly reduced and they have slipped a few places in rankings for a key term since. I found this bizarre as the last few links I built were both relevant and strong but I did use an anchor text? Any feedback would be useful, I'm slightly confused here?

Sitewide logo footer link - what's the risk?
Hi, an incredibly popular website, with several thousand pages, has offered me a site-wide footer logo link. The site this popular website would backlink to has 50 high quality backlinks (and low volumes of traffic - it's a new site). I am tempted to say no, because of the risk of penalty, but then I started wondering whether a logo link posed the same penalty risk as a text link.

Manual Penalty Removed - Recovery Times...
Howdy Mozzers, For anyone who has had experience of a manual penalty i'd appreciate your feedback. How long did it take to recover from a Manual Penalty? Of course every situation is different and its only been 8 days so perhaps it's to soon. Below is the email we received, I highlighted "believed" they didn't state we had. We highlighted a bunch of back links we didn't like however most of these remain in our profile in GWT so not sure what was really the problem. "Previously the webspam team had taken manual action on your site because we believed it violated our quality guidelines. After reviewing your reconsideration request, we have revoked this manual action. It may take some time before our indexing and ranking systems are updated to reflect the new status of your site." Your feedback would be greatly appreciated.
