I think that Google incorrectly flagged my href lang return tags as missing. What do I do?

Hi there SEOs and other respected online marketing enthusiasts!
Since a short while Google Webmaster Tools is flagging my href lang return tags as missing. However, these tags are available in the html and there should be no issue whatsoever. I often see other bogus flags raised by Google but this one is bothering us in particular. This is the code that we use for all our 18 languages:
All pages link to all other languages variables.
Is there something I'm missing? Or is it just an error on Google's site - and if so, how do I flag it as fixed?

What I would try out, is including a self reference also.
So next to your hreflang there would appear a new hreflang, such as:
When we have implemented hreflang, we have also used a self reference, and I do not have any issues there.

This is the holidaycars newyork page for the german language.
That tool is great, but i'm not sure how this occurs: the "Hreflang attributes don't match ( != de)"

Hi,
Try out your page with Flang tool. Maybe it will give a better insight what you are missing out.
We cannot do more without a link that we can inspect
BTW: common mistake, do you have a self reference? It is needed in order to hreflang to work well.

Does anyone know if it is possible/recommended/not recommended to use href lang in image or video XML sitemaps? This had not crossed my mind until recently, but a client asked me this question and I couldn't find any information on this topic.

Website No Longer Ranking In Google:
My website was on first page google couple of months ago, now nothing. Shows up in Bing page one. Some queries/pages still showing OK, but some not at all. Example "residential elevators illinois" found nowhere. http://www.accesselevator.net is the website. Have found 900 poor quality links and used disavow tool. Any further suggestions? How does google count a menu on each page
