Issues with Page Grader using Wordpress / Visual Composer?

HI I know there is a flagged issue with page titles on the Visual Composer - but it is telling me there is no test on one page completely even though there is??
And it works on the others?

Why do my Moz duplicate content results show me pages with no noticeably similar content?
Sometimes the "Pages with Duplicate Content" results under Content Issues show pages that, from what I'm able to see or otherwise test, have no duplicate content, save for the same navigation that exists on all of my pages. For example, a recent issue said that the following pages had duplicate content:

What does "Optimal Use of Keywords in Header Tags" really mean?
I get this explanation on the grader: Although using targeted keywords in H1 tags on your page does not directly correlate to high rankings, it does appear to provide some slight value. It's also considered a best practice for accessibility and helps potential visitors determine your page's content, so we recommend it. Over-using keywords, however, can be perceived as keyword stuffing (a form of search engine spam) and can negatively impact rankings, so use keywords in H1 tags two or fewer times. To adhere to best practices in Google News and Bing News, headlines should contain the relevant keyword target and be treated with the same importance as title tags. But I have the keyword in an H1 tag, only once. So why am getting this message on the grader? The URL is https://www.armsecuritysystems.com/business-security-systems/ The keyword is "business security systems" Any insight would be greatly appreciated.

Re On-Page Grader
One of the pages I'm trying to optimise is achieving an 'A' grade, however all the ticks are black not green as I've seen on other page grade. Why is this? Help much appreciated. Thanks

I have searched high and low for the source of this but just cannot find it. On-page grader gives the multiple page title elements warning, but I just can't identify where this is coming from. Can anyone, please, point me to the right direction?

Should I be getting an 'A' Grade for basic words in a page title?
It seems as long as a word in the page title matches a word(s) within the page content you will get an 'A' grade. Should I be replacing these keywords with higher monthly searches and lower difficulty? It appears deceptive when you test a new website and all the pages get an 'A' for a word as basic as 'about.' Please advise.

Duplicate Page Title query in the PRO Campaign tool
Can someone help me on this. I am seeing duplicate page titles on the PRO Campaign Crawl tool on an ecommerce site for example MOZ is saying that these two pages have a duplicate page title: http://www.cheapsnapframes.co.uk/colour-25mm-snap-frames/25mm-green-snap-frame/a0-traffic-green-snap-frame-25mm/prod_1730.html http://www.cheapsnapframes.co.uk/snap-picture-poster-frames/colour-25mm-snap-frames/green-25mm-snap-frame/a0-traffic-green-snap-frame-25mm/prod_1730.html They are the the same product in two categories. When I view the source of both pages the this link is the same in the meta: <link rel="<a class="attribute-value">canonical</a>" href="[http://www.cheapsnapframes.co.uk/colour-25mm-snap-frames/25mm-green-snap-frame/a0-traffic-green-snap-frame-25mm/prod_1730.html](view-source:http://www.cheapsnapframes.co.uk/colour-25mm-snap-frames/25mm-green-snap-frame/a0-traffic-green-snap-frame-25mm/prod_1730.html)" /> So is there something else I need to have done to erradicate this or is it not an issue?
