I have used my keyword only 12 times but on-page grader shows it 40 times?

I hope you are doing great, kindly have a look at this page, my keyword for this page is "Sports Streaming Sites". The text has a length of 5344 words and I'm using my keyword in this post as follow:
1. Text Body: 12 times(including headers)
2. Alt tags: 10 times (20+ Images)
3. Title: 1 Time
4. Description: 1 Time
5. Permalink: 1 TimeThis is a total of 25 times in 5344 words, but when I check this page with moz on-page grader, it shows that I'm using this keyword 40 times and its stuffed, can anybody please explain it to me, I'm so confused and it'll be so nice of you.

In this case Moz, isn't the smartest with their tool. What they're doing here is just checking the total source code apparently for the number of keyword matches in there. But what this does is that it's also checking all your META tags in the head. In your case I see at least another 4 mentions in there from OG:Description, OG:Title and Twitter:Title and Twitter:Description.
Another 4 mentions are currently happening in your Schema.org at the top for Organization, that's where I just stopped looking for more instances as they're probably just hidden somewhere else in either ALT tags or other snippets.
So I would, in this case, go with your own calculations for just using the number of mentions that is on the page and ignore the number that Moz is reporting here. Hopefully, Moz will change this over time and only check the occurrences in the actual BODY content.

Seomoz shows issue with canconical

SEOmoz tools: Keyword Difficuty + On-page Analysis
To analyse the current KWD situation. I would want to see the top 10 results link metrics (as in Keyword Difficulty & SERP Analysis) and each page's On page score (as in On-page Analysis) for the keyword. Those two figures would give me a pretty good picture of the current situation. Kind regards,

Twitter Page Authority Score?
I've been doing some competitive research in Open Site Explorer and many of our competitors have Twitter accounts very similar to ours. Their Twitter pages are usually one of the pages with linking to their website with the most Page Authority. The incoming links from Twitter are a "no follow" as you would guess. This has been the case for a large number of well ranking sites I have looked at. www.dremed.com also has a Twitter account at: https://twitter.com/#!/DREmed . However, Open Site Explorer does not list the Twitter link as an incoming link at all ( or if it does it has no Page Authority ). The Twitter account page seems very similar in nature to other competing Twitter pages. I'm not sure why it does not ALSO pull a high Page Authority score??? Do you know why this might be? How many times to repeat the exact keyword?
I'm curious what the suggested number of times is for including an EXACT keyword in a document, OTHER than in the page title, h1 and alt text? What does SEOMoz advocate for this? Bonus credit: What is the consensus on "stop words"? I've read the the search engines ignore these, yet searching for the same query with or without them produces vastly different results... So should they not always be included in one's keyword optimization, above?

My campaigns are not analyzing all my pages.
Hi I created a campaign against http://www.universalpr.com, and this campaign reports that only one page has been crawled. Keyword Rankings Question
Does the Keyword Rankings tool track all urls or just the main. Example: Say I put www.domain.com to have tracked. Will the Keyword Rankings tool also track www.domain.com/landing-page and any derivative of the main domain or would this be considered a separate campaign. Thanks
