In my keywords should they include the city name or not?
Example..... plastic surgery or plastic surgery Orlando
I know the search engine knows what city i am in, if i search for plastic surgery and i live in Orlando, does it pull up world wide plastic surgery links first or local plastic surgery clinics?

Yes, you should definitely include the city name. A search for just "plastic surgery" is too broad and much less likely to pull up your particular business.
When I type in "plastic surgery" in my search, not a single web page contains a local result for plastic surgery in the City I live in (although several of the paid ads show up locally).
Local Places pages do show up for local businesses, but I'm assuming you're talking about pages on your website and not Google Places.

Identifying Keywords - Longtail
I need some clarification as to how exactly keywords work. I learned that I should focus on more specifically targeted long-tail keywords. However I am uncertain of how this actually works. Say I decide to use the longtail keyword "online dyslexia test for children". Will my page only be found by people when they enter that exact search term or will it also be found when people search for any of the other terms containing the same words? International Keyword Ranking
I want measure my ranking for keywords in various countries and search engines. If I use a tool such as rank checker to determine my rank for google.uk, google.au.com, etc... is that accurate, or does my IP still affect the rankings I see? If I use a VPS such as Hide My Ass (or another product you would recommend), would that make my ranking results more accurate? I also want to measure my ranking in Russia, where Yandex is very strong so a tool to measure my Yandex ranking + Google would be ideal. What method you would recommend to accurately determine my rankings in different countries? Thank you!
