Meta tag "revisit after" - useful?

Hi everybody,
I've rarely seen the "revisit after" meta tag during the last 1,5 years.
As some of my current client websites are still using it and I'm not sure, if it's still usefull/has any effect, I'd like to hear from the community. Any advices/hints/experiences with the tag?

Use sitemap to give the SE's this hint, i say hint because that is all it is,

Alt tags advice
Hello, When describe an alt tag from what I understand you need to put your keyword when you can and not literally describe the image. For example let's say I have a picture with 2 cyclists on a road in the dolomites. I would tend to describe it saying " 2 cyclists riding in the dolomites" But for google it is better to describe it saying "Bike riding in Italy and through the dolomites" based on the fact that I am looking to rank on "italy bike tours" Isn't it ? Keyword density or No. of Time keyword used
Now, I know that there is no set figure to be used here, whichever metric you are using and it will depend on the article and what is natural. However, lets suppose for a minute that we are taking a keyword in isolation, and I have a 2000 word article using the keyword 17 times and rank no. 3 in Google SERPS. The no. 1 slot uses the keyword 8 times but only has a 800 word article and only a B grade on the onpage ranker. Of course, there are off page factors as well, but just wondering what your thoughts are on whether you look at density or total keyword usage. It is easy to just write without think about keyword density or usage, but occasionally you end up using the keyword about 50 times, and it is then I have to actually think about it. Other articles I barely use the keyword because the article just writes itself and it works out fine, but these are generally shorter. With longer articles on my best converting pages, I can't help but think about it more and it ends up a little hit and miss.

How "Top" or "Best" are considered when in front of keyword
I would like to know if someone has proven info how google today counts words "Top" or "Best" when in front of main keywords you try to rank for. For example, if I have a keyword like "Restaurants in Madrid" and I optimize that page without using words "top" or "best" will it have good rankings for keywords "top restaurants in madrid" and "best restaurants in madrid" ? I suppose that google is smart enough to know that web page should be good ranked even without using those 2 words but would like to know percentage of my loss if I just exclude those words from title tag and other important onpage factors. I want to rank high for all the 3 combinations, with "top", with "best" and without it in front so searching for best solution. I plan just to add one of those words, for example "top" and hope that google will know that "top" = "best" 🙂

Would This Be Highlighted as Duplicate Meta Desc?
Due to the nature of our business we literally have hundreds of web pages and so we recently asked our Web Developers to create the Meta Description for many of the pages that they create - unfortunately they don't seem to understand SEO but before I go back to them I wanted to run this by the MOZ Community... Is it impossible to get out of Panda? Matt Cutts says if you fix the problem you "pop back" but if so why are their so few examples?
In this video matt cutts says: http://www.youtube.com/watch?v=8IzUuhTyvJk about 15 "once we re-run our data (every few weeks) if we determine your site is of higher quality you would pop back out of being affected" Panda has effected thousands of sites and a lot of smart people have been working on the problem for about 2 years since the first panda was launched, but I can only find 1 site that has "popped back" to their original rankings. e.g. http://searchengineland.com/google-panda-two-years-later-losers-still-losing-one-real-recovery-149491 Apart from Motortrend.com I can't find any sites (of reasonable size) / case studies of sites that have solved the panda problem, and were definitely hit by panda. Which doesn't feel right, some people have deleted a ton of pages, redesigned their site, improved their content, etc with no success. Therefore is it a pointless exercise? Therefore, is it better to simply give up and start a new site?

Advanced tags in images
Is it true that one can improve image seo by adding metadata to the details (right click image and choose details), like tags and comments. I never heard of doing this before. It seems like grasping at straws.
