Keyword order on location searches

If someone is trying to find a service in a certain area they might use Google to search for "London plumber", "Plumber London" or "Plumber in London". Is there any research to show which of these tends to be used most, or are they all roughly the same? Thanks!

Getting People to Leave Product Reviews, When they've reached our site in search of reviews
We have a product review website that has been showing a steady increase in organic traffic. However we seem to be having a disconnect between what happens when a visitor arrives. We essentially want a visitor coming to our website in search of a review, to LEAVE a review for another product they may have used... Any suggestions on ways we might encourage this? Huge difference in search results depending on the country?
I have some keywords ranking in pages 2 and 3 in the US but for instance, in Canada, the same keywords rank somewhere between pages 15 and 25. When I check my US rankings, I use a Windows VPS located in the US. When I check my rankings in Canada, I use a computer physically located in Canada. Basically, about 80% of my rankings have gone away for countries other than the US. I have not set any geographical target for my website in GWT. How come there is such a big difference in search results across different countries?

How do I start to target a national audiences? I currently get most of my hits from The US and Australia? But I see no reason why I don't get vies in Canada and The UK.

Would you say it is more bennificial to seperate keywords in the title tag tag of a page using a common ( keyword , keyword | Domain.com) or using a hyphen as SEOmoz best practices reccommends (keyword - keyword | domain.com)?
Title tag best practices according to seomoz is the following keyowrd - keyword | brand.com but I have seen some interesting results from using a comma as to a hyphen to seperate keywords as reccomended and wanted to know which method is more crawler friendly.

Website coming up in omitted results for search term?
Hi, My website is coming up in omitted results for search term. Now I have heard before if this is the case then its mainly due to a duplicate content penalty. But the content I used is unique and produced by a copywriter so its good. Its a new site and was in the results for about 2 weeks, I changed the non www to www in webmaster tools 1 day ago and now its sitting in omitted results. In webmaster tools I had to add the domain twice one non www and one www. Both accounts I hooked up a sitemap. Is it just because the site is new and its just working out where to rank me? Just so you know the non www was indexed for the 2 weeks changed it in webmaster tools so I wanted the www one indexed and now gone into omitted results. Cheers
