Help.. there was a html and php version of my home page on my server for about a week. Now lost all rankings!

Our home page has disappeared from the Google SERPS completely. It won't even show for a search of our business name although other internal pages appear. Site appears in bing for our keywords but not for our business name.
When I republished the site I got a message saying there was a php and html version of the site on the home page. I deleted this immediately but was then advised by another staff member that they had also got that message the week before but not done anything about it - so I think it was out there for about a week.
Could this be the cause of the ranking drop? We have also done the following:
Checked GWM for robots issue, manual action, crawl errors, blocked URLs - all good. Some crawl errors on other pages relating to broken links. All sorted and marked as fixed.
Checked for duplicate content - redirected non www version of site to www
Checked back links
Removed video from home page that was temporarily unavailable when we did fetch and render
Added canonical tag
Added H1 tag that was missing
Coding checked - all looks good
There has been no change to the content of the page.
We have fetched and rendered and resubmitted for indexing but it's still not coming back in SERPS it did for a short time the other day and then after a couple of days disappeared again Prior to all this it had ranked really well for our most important keywords for years. It's not a new site.
The page is indexed as it comes up when doing a site: search.
Please can anyone help? Nobody seems to have the answer and we don't know what else to do. The site is

Hello again. I have just discovered this URL on a duplicate content checker site...

I have no idea where it has come from or how to get rid of it but it brings up my home page. Can anyone advise on this?

Thanks Paul. My first thought was to do a 301 redirect which I did last week on the index page but when the SEO company came out they took it off. They said I was creating a loop or something along those lines. I wasn't 100% sure what they meant. I didn't quite follow. Do you know what they meant? I created it in the same way as other 301 redirects I have on the site and they are all working just fine.

I'd strongly recommend changing that to actually redirect the /index.html version. There's no reason for either bots or human visitors to ever visit that URL. (A redirect is a directive, whereas canonical is only a suggestion and will leave the unwanted page in the search index.)

Thanks so much. I have now put a canonical tag on the index page to tell it to go to the home page. All of these things must help. Really appreciate the help from everyone.

Your home page is still also reachable under the /index.html version - that should be redirected to the real home page (the bare domain). In addition, the PHP version should probably have been redirected to the real home page, instead of just leaving it to 404, if it had ever been indexed by Google.
These aren't likely to be the primary reason for all versions of the page disappearing from the SERPs entirely, but they need to be fixed and then can be eliminated as a contributing cause.

How do we decide which pages to index/de-index? Help for a 250k page site
At Siftery (siftery.com) we have about 250k pages, most of them reflected in our sitemap. Though after submitting a sitemap we started seeing an increase in the number of pages Google indexed, in the past few weeks progress has slowed to a crawl at about 80k pages, and in fact has been coming down very marginally. Due to the nature of the site, a lot of the pages on the site likely look very similar to search engines. We've also broken down our sitemap into an index, so we know that most of the indexation problems are coming from a particular type of page (company profiles). Given these facts below, what do you recommend we do? Should we de-index all of the pages that are not being picked up by the Google index (and are therefore likely seen as low quality)? There seems to be a school of thought that de-indexing "thin" pages improves the ranking potential of the indexed pages. We have plans for enriching and differentiating the pages that are being picked up as thin (Moz itself picks them up as 'duplicate' pages even though they're not. Optimized Page Not Ranking for Head Term
We have a primary donation page that we've tried to position well for the term 'Donate' and some of its longer-tailed derivatives. The page has plenty of high-quality backlinks and internal links, and hasn't had any manual actions taken against it. The backlink profile is fairly good from what I can tell, definitely not mainly spam. The issue is that it doesn't rank for 'Donate', at all really. For 'Donate Online' and 'Donate Canada', it ranks roughly #3-4 across major Canadian cities, but we're not even in the top 100 for 'Donate'. There are pages and domains that are way less optimized, with much weaker backlink profiles, that are ranking well ahead of us. It's not a noindex or robots problem, as the page ranks fine for many other terms. We also have a strong domain with around 660k backlinks according to MajesticSEO. Here's the URL in question: http://www.redcross.ca/donate I'm hoping someone can help us diagnose what could be going wrong with this term specifically and how we can get this page into the SERPs where it belongs. Updated Wordpress version and installed SEO plugin, ranking went down
Last week, we updated our Wordpress version to the most recent version, and installed the SEO plugin. Our rankings immediately went down from #9 to #19. Is this normal? Will our rankings improve once the search engines crawl our site in the next couple weeks? Any feedback is helpful!

Canonicals for product pages - confused, anyone help?
I have an ecommerce website (built using Magento), and have just had the functionality extended to allow me to define my own canonical URLs. Currently the URLs are www. domainname.com/product-name.html but I can now change this to www.domainname. com/product-category/product-name.html. I was led to believe that this would be good for SEO. However, I have since had conflicting advice - it's been suggested that any links across the website that link to domain/category/sub-category/product will pass weight and authority through to the specified canonical anyway. Plus longer URLs are generally worse... I'm confused. Is it worth changing them? If so, would it be a bad thing to change all 700 canonical URLs at once?

Please help with page
We used to use this page http://www.discountbannerprinting.co.uk/banners/vinyl-pvc-banners.html to rank for the words vinyl banner and PVC banner but we have tried to focus the page only on PVC banners and move the vinyl banners word to http://www.discountbannerprinting.co.uk/ yet for some reason even though they have both been spidered google has now chosen to rank this page http://www.discountbannerprinting.co.uk/stickers/vinyl-stickers.html for the vinyl banner words- how do I stop this from happening I thought the home page would be powerful enough to rank for the word with a title inclusion and a spread of the word on the page. Deep Page is Ranking for Main Keyword, But I Want the Home Page to Rank
A deep page is ranking for a competitive and essential keyword, I'd like the home page to rank. The main reasons are probably: This specific page is optimized for just that keyword. Contains keyword in URL I've optimized the home page for this keyword as much as possible without sacrificing the integrity of the home page and the other keywords I need to maintain. My main question is: If I use a 301 redirect on this deep page to the home page, am I risking my current ranking, or will my home page replace it on the SERPs? Thanks so much in advance!

Pages On Subfolder Not Ranking
A subdirectory/folder on our website doesn't seem to rank for any keywords where the same type of pages on the same competition level keywords rank perfectly fine. For awhile the pages weren't getting indexed but were crawled regularly. Can't seem to figure the problem out.
