nofollowing no longer works, and although Google can read some javascript, you can obfuscate the js links and conserve pr from leaking that way

I say absolutely yes if you get on a page like this:

that page is PR5 and the links are dofollow. Try to confirm with them the page you will be placed on before paying. I may be doing this as well.
If joining allows you to use their logo as a trust factor on your site and you are in good standing then that is an additional benefit as well.

i guess what I am mostly looking for an answer about is which is better for rankings, the keyword in the folder or file name and if duplicate file names will harm rankings.

Assume there will be enough content on these pages to not get hit by panda.
The reason for doing this is to hopefully secure more than one first page result since these are names and very low competition, we see some sites doing this successfully.
We will have locations pages too which will list all the docs in that city

that sounds like he is willing to take it down but doesn't know how to? if that is the case i would work with him to get access to that account and delete it. Even if you have to offer to pay him a few bucks via paypal it's worth it if it's ranking well and getting hits - and it must be if it is ranking #2 for your company name that' huge. Get it removed it's costing you business.

I've got a question regarding URL structure. What is best for SEO given that there will be millions of lawyer names and 4 pages per lawyer

Also, concerning duplicate file names:
In the first example there are 4 duplicate file names with the lawyers name. (would this cause Google to not index some)
In the second example there are all unique file names (would this look spammy to Google or the user)
In the third example there are millions of duplicate file names (if 1 million lawyers then 1 million files called "office-locations" etc (could so many duplicate filenames cause ranking issues)
Should the lawyers name (which is the main keyword target) appear in the filename or in the folder - which is better for SEO in your opinion? Thanks for your input!

RE: Domain.com and domain.com/index.html duplicate content in reports even with rewrite on
a) request removal of the /index.html URL in webmaster tools and it will go away in Google's index quickly.
b) make sure that when you link to your homepage on your site you are not linking to the /index.html URL - I bet you are somewhere
do a sitewide search in dreamweaver to find all instances and do a global replace.

Since the adult site will most likely wind up getting links from other adult sites and similar shady places, you'll be asking for trouble even if it's a brand new site you're setting up eventually it will naturally gather links even if you do not actively seek them out - so I would not associate your non adult site with your adult site.
