OSE is not an end all be all for links. They only gather a certain number links and data.
You're already using screaming frog to get this data, so that should be accurate. You can also check Google Webmaster Tools, which has a section called "Internal Links".

Won't make a difference unless its within the body (my opinion).
I would also take note that the testimonials on your work/project page shouldn't be the bulk of the content on those pages. If they are just in a fixed location, that should be fine. Plus if they only appear the testimonial and work/project page, I wouldn't worry about it too much.

Google's fairly well at understanding pagination. You can either rel=canonical, rel next/prev or just simply leave them alone.
As for categories, i'd leave them as is, since you would typically want your categories to rank for the category keyword.

Noindexed pages should not appear in your "Index Status". I could be wrong but it doesn't make sense to appear there if the page is noindexed.
Doing a site:www.nyc-officespace-leader.com, I get 849 results. Seems normal to me. Again you would probably have to scrutinize your sitemap instead, sitemaps don't always pull all the URLs depending how on you get them.
Based on Screaming Frog, you got about 860 pages and ~200 noindexed pages. Your index status may update eventually.

Does not show up in SERPs. I wouldn't use Index Status as definitive but more as directional.

Index status is how many pages Google has indexed of your site.
Sitemap is different, incase your site has pages that are too deep for Google to find, sitemaps are created as a way to direct Googlebot to crawl pages that they won't necessarily find.
In your case Google indexed more pages than the amount of pages in your sitemap, which is absolutely normal.

A vanity URL is nothing extravagant. It is simply a URL that looks more appealing for marketing and user appeal. In your example, a vanity URL could simply be created by the website owner that wants to track the effectiveness of a TV campaign. The vanity URL can be a landing page or it can redirect to a "uglier" URL.
Vanity URL is more of a purpose based tactic and nothing special. You can make http://domain.com/vanity-name/ your URL and consider it a "vanity URL" or it can even be http://domain.com/a-witty-punchline-that-is-memorable/
To track it, just simply use an Analytics tool like Google Analytics or use a redirect.

RE: Canonical Tag on All Pages
There isn't a major negative effect when using canonicals even when they are not needed. Some CMS use sitewide canonicals to easier tackle duplicate URLs. So if a base URL is using parameters, the CMS might have been setup to follow back to the canonical URL.
A quick example would be: view-source:http://www.expedia.com/

RE: Meta Keywords: Should we use them or not?
Meta keywords are no longer being used by most search engines, especially Google. I suggest removing all meta keyword data because the only thing it does is it allows competitors to see what keywords you are trying to target.
Most popular search engines don't use this to rank anymore, and it is best to not do this.

Make sure you send in a sitemap and all should be well.
I've dealt with cases where certain pages were noindex but then removed. As long as you fixed all your errors, it should be back to normal. Think of a site going down intermittently, rankings don't get affected too much (I believe Matt Cutts confirmed this in a youtube video)

Referring pages is the amount of pages (URLS) pointing to your site.
Total backlinks is the total amount of links you have to your site.
Example: 10 pages have 2 links to your site.

Just because spammers use WP, Google doesn't view everyone using WP as spammers.
WP.org and WP.com are for different types of users and there are successful WP.com sites as well as WP.org and vice versa for spammers.
WP overall is a great CMS for blogging and managing content and I would recommend using them.
Just create awesome content, share, and create buzz, you will rank well.

Well I've finally gotten been able to get the penalty removed judging by this email:

This was after a reconsideration request was sent prior to disavow tool being released. In addition I also applied a disavow of all the links I was unsuccessful in removingwithout contacting Google and letting the original reconsideration request run it's course.
I am making this post just to let everyone know that the hard work pays off and Google is just trying to make sure you are doing your best in removing the links. As 'Ryan Kent' always emphasizes, you must really be diligent and honest when trying to remove links. You also need to keep documentation, I anchored contact pages and email addresses, 1st, 2nd, 3rd, and even 4th attempt dates.
Now with the disavow tool, I believe if you do a "good faith" in removing the links, and it is well documented, you can use the disavow tool after multiple attempts, correlating both the disavow links and the spreadsheet sent to Google is and should be very important in a reconsideration request.

Also I received the message from WMT, and wondering does anyone know how long is 'some time' before site is reindexed? So far our organic traffic is still about the same prior. So I would like to hear what other's experience are after a successful reconsideration.

Don't even need to look at their site. Best way to build link is doing it in house. Make fresh and intriguing content, and you will be surprised how much that will help.
Invest that $97 a month on building an awesome article/content(video) and you will get great backlinks. Its hard but once you get a hang of it, you will realize how easy it is, with the correct budget in building the content.
And once again, any of these "give me $10 and I'll give you 1 billion links" is just utter BS and bad for you and business.
This really makes our industry harder to teach individuals that it is not as simple as paying someone and they get you backlinks, and we have to reeducate them exactly why and what you should be doing instead of paying for links.
Also Google is known 100% to devalue and even penalize links in blog networks.

That is not the problem, Moz rank checker is often done at random times and at any given time a keyword could not be ranking.
I'm pretty sure Moz crawls pages with personalization off (pws=0). For instance I use to do freelance for http://www.canvaspaintings.com and they rank #1 for "canvas paintings", however sometimes they would not even appear in the top 100.
If Moz often gives the keyword the wrong ranking, you should follow up with them. If this happens maybe one or twice, it should not be a big problem. Just know that you are ranking.
