On-page score vs adwords quality score

Dear Sirs.
According to your on page SEO card my page gets an A grade. But in google adwords the quality score is 3. Why is that? What's the difference?

Quality score is determined by click through rate and relevance of the advert. The landing page quality is a factor but not the biggest factor in determining quality score.
Split your keywords into more targeted groups, make sure the ad is relevant to the keywords in that group and increase spend to get your quality score up.

Home page keyword in url
I have been looking into SEO for a few weeks now trying to perfect a homepage. Going through various sources on MOZ, and other examples out there on the internet, I keep seeing that you should have your keyword in the URL of the page. The homepage is the page most people want to rank the highest in google searches, however, you cannot put the keyword in the URL as most home page URLs are simply /. Should I actually make the home like this: www.example.com/key-word-example? I would imagine this would not be the normal for many users and would seem like it's not the home page.

Home Page Keywords not Ranking and Assigned to Inside Pages
Hi, thank you for taking the time to read this. We have a few websites with the same problem. I will use http://www.prepared-meals.com as an example: The home page was ranking on page one for keyword "Prepared Meals". The site is about 6 months old. We use the Moz page optimizer on all pages of our websites to score an A rating. Recently we found the home page is no longer showing up in search results and the keyword "prepared meals" now points to an inside page that is not relevant: http://www.prepared-meals.com/Senior-Meals/Moms-Meals-Reviews.html this page shows up for Prepared Meals around page 15 in Google results. We have read keywords in the URL might be the issue, even though the page optimizer in MOZ says to do that. We are wondering if this is the issue or there is some other problem we are not aware of. What automated tools produce a number score for A) On-Page Optimization and B) Domain Exact Match?
SEOMoz Community, -I currently use SEMRush, SEOMoz Open Site Explorer and Archive.org for the other Benefit and Opposition factors. -I’ve had to manually search pages for keyword use in the title, footer & body for on-page optimization. -I’ve also manually searched Google for Domain Exact Matches. Category listing page coming above product pages
A new SEO client we have taken on seem to be hitting most of the points right on with their site and SEO. However one thing that is bugging me is that their category pages i.e. "Footwear" which title tag includes the brands they stock. Is almost always coming up above (if they are ever even found) the product individual pages. Anyone seen this sort of things happening? Very frustrating.

Why home page ranks higher than keyword-optimized page
We have a page that is optimized for the keyword "job scheduling". A search on the keyword "job scheduling" results in this page not ranking at all, while our home page (uc4.com) ranks third.
