Image Alt tags--always include the targeted keyword?

Question for all the SEO's out there. Do you always include your target keyword in the image alt tag?
For example, if you had an article on osteoarthritis, and you included a photo of an old man, would you put "old man on a bench" or "old man suffering from osteoarthritis" -- even though you have no idea if the old man suffers from osteoarthritis?

Related-ish question:
I have a site-wide banner on a website with a theme related to mine with the alt tag containing an exact-match keyword.
It's on 2,500 pages. Post-Penguin, am I inviting trouble? Should I play it safe and have it on, say, just a handful of pages -- or even just the homepage?

As a photographer in a past life, what I did that did not work was use the same tag on every image in a post. That doesn't work.
What does work is variety. Use the keyword, use related words, and use descriptive image words. Say you had 3 images to keyword:
(image of an older guy on a bench) alt="osteoarthritis frequently affects elderly men"
(doctor, any health image) alt="healthy screenings can help reduce the symptoms of osteoarthritis"
(drug you're selling) alt="prevent joint pain and reduce osteoarthritis symptoms with X"

Agree with Davinia 100%. Write alt tags for the user - how would you BEST describe the image in a few words? I try to include a few keywords/phrases but if I have a page with 10 images I'm not going to include the same keyword(s) in every single image - probably around 50-80% of the images.

I would use the keyword in a keyword phrase and limit to 2-3 words, so something like "Man with osteoarthritis" or "Osteoarthritis affects everyone".
Remember alt tags are also used for usability, so if someone say has a computer that reads them the on-page content (e.g. a visually impaired person) you want your content to clearly and accurately explain what's on the page.

Ideal Frequency of my related keywords
Let's say on my keyword If I take the 1 st 20 results of google they have an average number of words per page of 3000 words. On my page I only have 1500 words. Does it mean that the frequency of my related keywords should be half of what the others have ? What about if I am over their frequency is it a problem also ? Thank you,

Invest in a Image Sitemap - Yes or No?
Hey Mozers, 2 part question I'm reaching out to see if you all think Image Sitemaps are totally worth it for a big company. I can totally understand its value for a smaller mom & pop company. With a larger company they would have way more products so is it worth it having an image site map? I cant find examples of image sitemaps online. Would you be able to provide a website that is doing it? I can only find video sitemaps.

Image impressions fall drastically

One word Keywords
Hey as you know that as a seo we are, we always optimize keywords which are at least 2 words, and lets say I'm trying to optimize a page for terms like "man clothing, man london clothing, man great collection, man stylus collection" and as you can guess I optimize this pages for this keywords by inputting them into title heading tags and body.

So my question is , what if google takes "man" phrase from my 2 words keywords, and pretend as a my keyword. (I mean what if google thinks my keywords is man because as you can see in all of the keywords "man" is in all of them.)
And what if Google thinks the density of "man" probably would be %20 which is astronomic number.? Sorry for my bad english.0 -
Worth removing keywords...?
I was just going over a site I manage and noticed it had a load of meta keywords on it. Probably 10-15 keywords per page... Do you think this is harming the site? Is it worth removing them?

2 Year Old Keyword Focused Site Will Not Rank for Keyword
Hi All, I need your help. This site is confounding me. The site is turnstilefactory.com It's a few years old. Strong domain name and seo focused on the term 'turnstile'. In bound links are not abundant, but certainly not absent either. Considering the subject matter, content and competition in the space, I would expect this site by now to at least be in the top 10 pages for the search 'turnstile', but it's not. I've tried everything I can think of with this, but it just won't rank for anything other than it's domain name. Can anyone please take a look and let me know if they see something I'm missing? It would be appreciated. Thanks.
