Ranking drop for a particular page on a particular keyword

Take a look at centerforhealthysex.com/sex-addiction.
For months we were ranking in the top 3 spots with this URL for "sex addiction los angeles", even reaching #1 for a while.
Late last year we redesigned and developed the site to clean up the code and redirects. We also cleaned up the internal linking structure. For years we had been ranking on "sex addiction los angeles" for the home page ... bumping around the top 5 spots, but we wanted organic traffic to go to /sex-addiction. In the Fall, we saw overall site traffic rise steadily. We made few changes to the site and none to this page or links flowing back to the page once we had achieved strong ranking -- we didn't want to mess with a good thing.
Then November 27th we started losing ranking on this term and a couple others. The good news is that we gained ranking on some high volume traffic terms so overall organic traffic is reasonably strong, BUT we're not ranking on the terms where we want to rank.
Centerforhealthysex.com/sex-addiction is now nowhere to be found on the target search term despite fairly strong page rank.
I tried redoing and resubmitting the site map, cleaning out some potentially duplicative content but to no avail.
I see no issues, errors or warnings in Webmaster Tools. We have a few medium priority fixer-uppers in SEO Moz, but we've taken care of the majority of the big stuff.
What am I not seeing?

Interesting... what are the other urls/keywords that have been affected? Maybe we can find a pattern.
I wonder if its a language filter causing this...

Organic traffic overall is fine, but this KW (and some other geo-local long tail terms) just keep falling in rank...

The fact that its for 1 keyword and 1 page leads me to believe that it is penguin. The refresh came on Oct 5th but could be related. Doesn't look like it has any problems with its backlinks so that leaves on page. I ran some keyword density analysis on the site and you have very high percentages for your content.
I don't usually recommend this but you should try decreasing the number of instances of your target keyword. Not by much (3 or 4 instances) but just replace several instances of "sex addition" in your body content to some synonym/different variation e.g. "addiction to sex", "the addiction", etc.
If you decide to take my advice, PLEASE let me know if it worked. This is a really interesting case
Edit: ran copyscape and see a bunch of results. I know that you are trying to quote a respected source but maybe G is taking offense to that? Although it generally shouldn't penalize you for quoting a small portion of your pages content, G could be treating this differently for one reason or another.

Keyword Ranking Fluctuations
Hi Guys I am currently working on a website where one of the keyword targets is fluctuating. The keyword is fluctuating between page 2 and page 5. What makes this strange is that we are not experiencing the issue with any other keyword targets. They are all ranking fine. It is only 1 keyword. The keyword target happens to be the main homepage keyword target - not sure if this makes a difference? The homepage targets 2 keyword e.g. Business Offices & Accessories. The homepage ranks perfectly fine for e.g. Business Accessories but is fluctuating for e.g. Business Offices! Very strange. What makes it even stranger - the keyword variations of the fluctuating keyword e.g. office for business - these variations are all fine and not fluctuating. Its only 1 keyword. If anyone has any ideas or feedback that would be great! Thanks, Duncan

Google no longer highlighting keyword times in titles within rankings?

Page position dropped on Google
Hey Guys, My web designer has recommended this forum to use, the reason being: my google position has been dropped from page 1 to page 10 in the last week. The site is weloveschoolsigns.co.uk, but our main business site is textstyles.co.uk the school signs are a product of text styles. I have been told off my SEO company, that because I have changed the school logo to the text styles logo, Google have penalised me for it, and dropped us from page 1 for numerous keywords, to page 10 or more. They have also said that duplicate content within the school site http://www.weloveschoolsigns.co.uk/school-signs-made-easy/ has also a contributed to the drop in positions. (this content is not on the textstyles site) Lastly they said, that having the same telephone number is a definate no no. They said that I have been penalised, because google see the above as trying to monopolise on the market. I don’t know if all this is true, as the SEO is way above my head, but they have quoted me £1250 to repair all the errors, when the site only cost £750. They have also mentioned that because of the above changes, the main text styles site will also be punished. Any thoughts on this matter would be much appreciated as I don't know whether to pay them to crack on, or accept the new positions. Either way I'm very confused. Thanks Thomas

Sudden drop in ranking for major search terms
Site bumpbabyandbeyond com au. Help! We have been operating for six years and had steadily built up our ranking for major terms like maternity clothes, maternity clothing, maternity wear, reaching highs of 6, 8 and 10 respectively for AU sites AU wide about six months ago that we have steadily maintained. All of a sudden we have dropped away. A week ago I noticed we had dropped from 6 to 12 for maternity clothes. This morning we are 21! I can't see any obvious reason for this, but believe the eCommerce module of our inventory/pos software has had a recent update - I'm awaiting answers on this. We haven't actively had anyone link building or working on SEO after being badly bitten and shelling out a small fortune for an AU company to do very little over six months - rankings improved rapidly when I sacked them and did some on page minor work myself. But I don't have the time or knowledge to look after the seo, and am on the hunt for reputable white hat assistance. Is there anything obviously wrong that I need to fix ASAP? Any help would be much appreciated 🙂

Transfer a page that is already ranked from a second to a third domain level
Hi everybody, i have a page well optimized for my keyword it is on a domain with 28 domain authority it is well linked but it isn't enough to rank first Do you think that buy a third level domain with the keyword inside, put the page on-it and re-index it is a good solution? ( by putting a 301 redirect on the previous page of course ) now i'm waiting to be indexed for the new page but nothing appear searching the keyword instead the previous ranked page... thanks Guido

2 Year Old Keyword Focused Site Will Not Rank for Keyword
Hi All, I need your help. This site is confounding me. The site is turnstilefactory.com It's a few years old. Strong domain name and seo focused on the term 'turnstile'. In bound links are not abundant, but certainly not absent either. Considering the subject matter, content and competition in the space, I would expect this site by now to at least be in the top 10 pages for the search 'turnstile', but it's not. I've tried everything I can think of with this, but it just won't rank for anything other than it's domain name. Can anyone please take a look and let me know if they see something I'm missing? It would be appreciated. Thanks.

Creating new pages for geo targeted keywords
What's the best way to go about creating new pages for geo targeted keywords for a business? I ask because, these keywords are areas that they provide service to, but the brick and mortar business is not located. I've already run into problems trying to put too many locations onto one page. What's the best way to attack content for these new pages in order to get these geo keywords in there?
