Ranking drop for a particular page on a particular keyword

Take a look at centerforhealthysex.com/sex-addiction.
For months we were ranking in the top 3 spots with this URL for "sex addiction los angeles", even reaching #1 for a while.
Late last year we redesigned and developed the site to clean up the code and redirects. We also cleaned up the internal linking structure. For years we had been ranking on "sex addiction los angeles" for the home page ... bumping around the top 5 spots, but we wanted organic traffic to go to /sex-addiction. In the Fall, we saw overall site traffic rise steadily. We made few changes to the site and none to this page or links flowing back to the page once we had achieved strong ranking -- we didn't want to mess with a good thing.
Then November 27th we started losing ranking on this term and a couple others. The good news is that we gained ranking on some high volume traffic terms so overall organic traffic is reasonably strong, BUT we're not ranking on the terms where we want to rank.
Centerforhealthysex.com/sex-addiction is now nowhere to be found on the target search term despite fairly strong page rank.
I tried redoing and resubmitting the site map, cleaning out some potentially duplicative content but to no avail.
I see no issues, errors or warnings in Webmaster Tools. We have a few medium priority fixer-uppers in SEO Moz, but we've taken care of the majority of the big stuff.
What am I not seeing?

Interesting... what are the other urls/keywords that have been affected? Maybe we can find a pattern.
I wonder if its a language filter causing this...

Organic traffic overall is fine, but this KW (and some other geo-local long tail terms) just keep falling in rank...

The fact that its for 1 keyword and 1 page leads me to believe that it is penguin. The refresh came on Oct 5th but could be related. Doesn't look like it has any problems with its backlinks so that leaves on page. I ran some keyword density analysis on the site and you have very high percentages for your content.
I don't usually recommend this but you should try decreasing the number of instances of your target keyword. Not by much (3 or 4 instances) but just replace several instances of "sex addition" in your body content to some synonym/different variation e.g. "addiction to sex", "the addiction", etc.
If you decide to take my advice, PLEASE let me know if it worked. This is a really interesting case
Edit: ran copyscape and see a bunch of results. I know that you are trying to quote a respected source but maybe G is taking offense to that? Although it generally shouldn't penalize you for quoting a small portion of your pages content, G could be treating this differently for one reason or another.

Home page ranks for most keywords with no SEO
Hi Guys, The keyword I am trying to rank is on seperate page with its own Optimization in place. However the HOME page of website starts ranking for that keywords . The keyword is mentioned on Home page Just once in content description, that's all. What should be my ideal strategy. Deleting the Sub Page, will that improve the SEO of my home page and improve the Rank for that keyword ? Also I can see my Own YELP pages rank better than the actual website for few terms . Number of indexed pages dropped. No manual action though?
I have a client who had their WordPress site hacked. At that point there was no message from Google in webmaster tools and the search results for their pages still looked normal. They paid sitelock to fix the site. This was all about a month ago. Logging into Webmaster Tools now there are still no messages from Google nor anything on the manual actions page. Their organic traffic is essentially gone. Looking at the submitted sitemap only 3 of their 121 submitted pages are indexed. Before this all of them where in the index. Looking at the index status report I can see that the number of indexed pages dropped completely off the map. We are sure that the site is free of malware. This client has done no fishy SEO practices. What can be done?

New Website Launch - what to do with the URLs of the pages with ranks
Hey there, So, we are "redesigning" our website, it will have a new user journey and overall layout, use, and feel. Situation: Previously, most of our keywords ranked over time organically though all of them pull up our domain.com as the landing page. Now that we are redesigning the site, most of the keywords pointing to the home page will now have their own page. Keywords properly grouped and content will now be on topic and focused per page. Q: What are the things that we need to do so we won't lose those keywords? Appreciate your help. Also, if you can cite specific SEO checklist when redesigning a site, that'll be a great help! Thanks! Help! The website ranks fine but one of my web pages simply won't rank on Google!!!
One of our web pages will not rank on Google. The website as a whole ranks fine except just one section...We have tested and it looks fine...Google can crawl the page no problem. There are no spurious redirects in place. The content is fine. There is no duplicate page content issue. The page has a dozen product images (photos) but the load time of the page is absolutely fine. We have the submitted the page via webmaster and its fine. It gets listed but then a few hours later disappears!!! The site has not been penalised as we get good rankings with other pages. Can anyone help? Know about this problem?

Meta Keywords: Do They Hurt Rankings
I know that Google doesn't use meta keywords, but does it hurt to have anything in there? Just wondering if I need to remove all the meta keywords that are on my site, or are they harmless to have. Also, would meta keywords ever be used by an internal search plug-in if we were to install one in the future, or do they generally look at the product title and description for that info?

Optimize Pages for Keywords Prior to Building Links?
Greetings MOZ Community: According to site audit by a reputable SEO firm last November, my commercial real estate web site has a toxic link profile which is very weak (about 58% of links qualified as toxic). The SEO firm suggests than we immediately start pruning the link profile, requesting removal of the toxic links and eventually filing a link disavow file with Google for links that web masters will not agree to remove. While removing toxic links, the SEO firm proposes to simultaneously solicit very high quality links, to try to obtain 7-12 high quality links per month. My question is the following: is it putting the cart before the horse to work on link building without optimizing pages (with Yoast) for specific keywords? I would think that Google considers how each page is optimized for specific terms; which terms are used within the link structure, as well as terms within the meta tags. My site is partially optimized, but optimization has never been done thoroughly. Should the pages of the site be optimized for the top 25-30 terms before link building begins. Or can that be done at a later stage. Note that my link profile is pretty atrocious. My site at the moment is receiving about 1,000 unique visitors a week from organic search. However 70% of the traffic is from terms that are not relevant. The firm that did my audit claims that removal of the toxic links while building some new links is imperative and that optimization for keywords can wait somewhat. Pages On Subfolder Not Ranking
A subdirectory/folder on our website doesn't seem to rank for any keywords where the same type of pages on the same competition level keywords rank perfectly fine. For awhile the pages weren't getting indexed but were crawled regularly. Can't seem to figure the problem out.
