Ranking for competitive keywords

Hi Folks,
I am relatively new to SEO and I was hoping folks here could give me some guidance/tips on ranking in a competitive keyword space. My client is a health care provider and they wish to rank for terms like 'heart attack' which I believe will be quite difficult due to it being a short tail keyword and it is a very competitive space. Any an all advice and input is greatly appreciated.

Starting from scratch to rank for a term like "heart attack" would be quite an endeavor for a seasoned SEO with a client who had a fairly substantial budget, let alone for someone new to the discipline. Not that it can't be done, but knowing enough to inform the client what it will take and what might be more productive options for their practice is part of being and SEO. It sounds like you're working on coming to grips with that knowledge at this very point.
Starting out, I'd recommend a couple of things: Do an advanced Open Site Explorer report on your client's site and then do a comparative OSE report against some of the other sites that rank on page one for that term. Then do another report comparing your client with other sites even further down in the results to get a sense of where the client might be starting from and what they would need to do to get to the top. Installing the Mozbar can be helpful too.
Moz has another tool that you might find handy--it the keyword difficulty tool and be sure that you've read through their How To Do Keyword Research guide, as well.

So your goal is:
to rank on high competitive short-term keywords in a stable position and over a longer periode, right?
There are 3 possibilities (which you won`t like):
1. your client has a fortune to spend for SEO and SEA
2. you can use black hat techniques
3. your client has patience (a lot of)
The last one is the most realistic and MAYBE (there is no guarantee!) the most effective way. You have to do a lot of on-page and off-page work to establish the authority of a new domain for a few of keywords. You have to prove that your site has the relevance to be an authority for e.g. "heart attack" - this process takes some time.
We needed several years to make clear that our website is the authority for the keyword "guitar" (the german word for it) within Google Germany - the result is a stable and constant high position for years as well (no.1)... but the list to (maybe) be successful with that is long...
I would always recommend to optimize a long-term keyword which is not so competitive. If you are successful to establish to be an authority for such keywords will help you for your next step: to become an authority fpr high competitive keywords ... try to make your customer clear that you can bring them on top very fast but that they will also fall down much more faster then!
It`s just like buidling up a good reputation or to establish a good credit history - but I believe you already know that!

Will Google Judge Duplicate Content on Responsive Pages to be Keyword Spamming?
I have a website for my small business, and hope to improve the search results position for 5 landing pages. I recently modified my website to make it responsive (mobile friendly). I was not able to use Bootstrap; the layout of the pages is a bit unusual and doesn't lend itself to the options Bootstrap provides. Each landing page has 3 main div's - one for desktop, one for tablet, one for phone.

The text content displayed in each div is the same. Only one of the 3 div’s is visible; the user’s screen width determines which div is visible. When I wrote the HTML for the page, I didn't want each div to have identical text. I worried that
when Google indexed the page it would see the same text 3 times, and would conclude that keyword spamming was occurring. So I put the text in just one div. And when the page loads jQuery copies the text from the first div to the other two div's. But now I've learned that when Google indexes a page it looks at both the page that is served AND the page that is rendered. And in my case the page that is rendered - after it loads and the jQuery code is executed – contains duplicate text content in three div's. So perhaps my approach - having the served page contain just one div with text content – fails to help, because Google examines the rendered page, which has duplicate text content in three div's. Here is the layout of one landing page, as served by the server. 1000 words of text goes here. No text. jQuery will copy the text from div id="desktop" into here. No text. jQuery will copy the text from div id="desktop" into here. ===================================================================================== My question is: Will Google conclude that keyword spamming is occurring because of the duplicate content the rendered page contains, or will it realize that only one of the div's is visible at a time, and the duplicate content is there only to achieve a responsive design? What might be exact reasons to vary rankings in countries
Our website ranking always fluctuates from US and India. Our primary visitors are from US followed by India. We can see 10 ranking positions difference in these countries whenever we did some changes. Why this? More precisely to say, rankings are inversely proportional like rank drops in other country while improved in one country and vice versa. What could be the possibility? We been facing this from an year and searching for someone who could really make a move on it.

Lost Rankings Late April Even Though We Have A Mobile Site
I have noticed a significant drop in rankings since late April. It is about a 30% drop in organic from Google. This is despite the fact that we launched a mobile site before the update. What gives? Any thoughts or suggestions would be appreciated.

Yoast focus keywords for portfolio post types in WordPress
This one is for the WordPress optimization crowd! Portfolios are used to display work. I have a question about best seo optimization techniques. 1. Portfolios can be used to display many different types of work, for me it either original web designs from scratch, WordPress redesigns, or importing a current website into WordPress. What is the best practice for keywords for multiple portfolios that are in one category. for instance WordPress Redesign. If I have 5 WordPress redesign portfolio posts is it good practice to use WordPress redesign for all 5 portfolios or should they use variations? Yoast gets angry when the same focus keyword is used multiple times. 2. Should portfolios even be indexed? Since this is how I attract new customers I would think yes but am I giving too much exposure to my client and not enough to my business. I guess this will depend on titles and meta descriptions. A discussion on best practices here is what I am really looking for. What is your advice and opinion on the matter.

Non-performing keyword
We recently completely started over with our website. I didn't mind not ranking for these keywords for a while because through extensive TV commercials our branded keywords perform the best for us anyway. However, a month and a half later, all but one keyword is ranking in the top 3 pages on Google. We are using Joomla! CMS and I first suspected that it may be a problem, but all of my crawl results are good. I think Joomla sites look good and are really easy for clients to edit on their own. I would really like to keep offering Joomla sites, but if this is the cause, I may need to reconsider. I have a few links to the page and I do know that I need more, but I have pages and keywords with fewer backlinks and harder keywords that are ranking. I have been busy with clients and unable to spend a lot of time on MY site ( My grandma always told me, " The Shoemaker's kids go barefoot"). I'm wondering if anyone can take a peek and help me over this hump? It's probably something stupid, and the rest of my sites are performing on the 1st page for their keywords, so I must be doing something right 😉 . I'm just not sure what I'm not seeing. Rankings disappears for two days
We have noticed with 3 of my sites that rankings totally disappears for 2-3 days and then appear again on google 1st page. Why does this happen?

How would restructuring the navigation of my website affect my rankings?
I want to restructure the navigation of my website for a few reasons: 1. It isn't intuitive/clear to the user 2. It is way too big, it has too many links and thus causes the number of links on many pages to be >100. 3. I want to get rid of file extensions as part of the URLs (.html, .php) 4. I want to achieve a "tree"-like navigation system, with categories, subcategories and so on. In the process of cleaning up my website, I had to 301 redirect a lot of duplicate pages, fix broken links, etc. I have a lot of 301 redirects already, and in the process of restructuring the navigation of my website I know I'm going to get more. Will the addition of new 301 redirects have an effect on my rankings? (I'm basically going to be changing all of the URLs) What kind of SEO effect will restructuring the navigation at the top of the page (reducing the # of links on the main menu) have on my site? What is the best strategy to implement in this situation?
