Creating Listings For Doctors and Dentist aside from the Business Name

What is the best way to create listings for Doctors/Dentist and keep them from showing as duplicates. Should they just have there name without the company name in the title but yet the address and same phone or no separate listing at all.
I have noticed in Moz Local that these show as duplicates when listed in this format.

I wouldn't recommend tracking or redirecting phone numbers, actually. Google has historically had issues with both. The unique phone number is ideal, but I understand it's quite uncommon in medical practices. Normally, all calls are answered by the front desk in medical practices, so this may not be a safeguard you can comply with. If your clients were realtors or insurance agents, it would be more typical for them to have their own phone numbers, but doctors don't usually seem to. So, the upshot here is going to be that you're likely to have to use the same number on the listings and cross your fingers that the other areas of differentiation will help Google keep the various practitioners separate.
Google is better than in the past about preventing merges, but they still happen, so you'll always want to keep an eye on the listings and if you notice details getting mixed up or discover a sudden drop in rankings or something along those lines, you'll need to investigate the presence of merging and duplication. Likely - it won't happen - but it's best to be prepared:)

If they don't have separate lines but rather extensions I guess you really can't put extension in listings. I guess maybe a tracking number would be ok in this case forwarded to the regular number even though you don't recommend tracking numbers for listings?

The best way to differentiate practitioners in the multi-practitioner scenario is to aim for as many of these as you can:
1. Put only the doctor's name in the business title. So, Dr. John Johnson - not Dr. John Johnson, Greentree Dental
2. If possible, list a unique phone number for each doctor/dentist at which he/she is directly contactable. I know this isn't always possible, but if it is, it can reduce the risk of merging.
3. Do not share Google My Business categories between the practitioners. If possible, divide the chosen categories between the practitioners.
4. Set up a unique landing page on the website for each practitioner and point the Google+ Local page to it, instead of pointing them all to the homepage. Be sure the complete contact info for the practitioner is on this page and the content is unique.
5. Write a unique Google+ Local description for each practitioner.
All of these precautions can help reduce the chances of accidental merges and duplications.

Our Google my business listings aren't syncing with Moz listings. I've tried importing them but nothing happens. Anyone know what the problem could be?
We have a professional level subscription for 5 company listings on Moz Local. These listings seem to have synced everywhere except for Google My Business. 1 out of 5 of these synced with Google My Business. When I try to import the listings that aren't synced, nothing happens. I can't add a new listing to Moz Local
Hi, Ive been trying to add new listings for two clients to Moz Local for four days now. They each have their own login, so I don't believe it is an account issue. I have rebooted, tried different browsers, cleared cache, and even tried a different computer, all to no avail. Here's what happens... I login and click on Add Listing. I search for the business, select the best result and then I just get a bouncing map pin. It never goes any further.

Moz Local, listings still not correct
I've been using Moz Local on all my clients. One client in particular, I've been using Moz Local on for 6 months. Their Moz Local stats all look good inside the dash board. While killing some time the weekend, I ran a few reports on them in Bright Local. None of the sites Moz Local is supposed to be pushing too has the correct listings. They are all different just like before I started. Why is this?

Facebook listing not found
Kinda new to Moz Local. Up until about 5 days ago, I was getting 96-97% reported by Moz Local - then suddenly that dropped to 84% and Moz Local reported that my Facebook listing wasn't found. I'd seen face book come and go, sometimes Moz Local would say FB couldn't be reached but usually it would show up later that day and I'd get 96% again. But now its been about 5 days and FB keeps showing up as no listing found. I completely retyped in my FB url and verified everything but still Moz Local won't update.

Can MozLocal cause duplicate listings?
I had already claimed many of our listings before subscribing to Moz Local, will Moz Local just update those claimed listings or does it use the CSV file to create a duplicate listings? Out of nowhere I had about 5 duplicates pop up and am not sure where they came from. They are a beast to get rid of so hoping there is an easy explanation/fix!

Google+ & Yelp Listings not recognized by Moz Local
I've been attempting to get a client's local listings all cleaned up using Moz Local. When I first did a scan, the Yelp listing was there but with an incorrect address. I uploaded all the data via CVS file successfully about a month ago. I logged into the claimed Yelp profile as the owner and made my adjustments. Now, the Moz Local tool says there is no Yelp listing and it's also reporting that there is no G+ page. Can anyone explain? Thanks!
