Duplicate Title Tags/Meta Tags for Website with Multiple Locations

I currently have an insurance website that has over 40 offices in Ontario. The site also provides online quoting.
In some of our programming, we have implemented variables in the URLS that will allow the quotes to go to the specific city offices - the issue I am having is that the quote in itself is the same quote form (same title, same meta) because it's technically one page on the website. We did it this way to avoid having to update 40 forms if a field on the form were to change. Is there any way I can relieve my site of this duplicate title tag/meta tag issue?Any insight would be really appreciated - thanks so much!

Are individual versions of the quote form being indexed? Do they come up in a site: search of the website? Are individual quote form URLs serving as landing pages for organic traffic? If not, you are probably good to go. It doesn't sound to me like your current situation is much to worry about.

Each location has it's own individual page (and all URL's are unique), but all locations share the quote forms through parameters. Each quote page already has canonical implemented and I've reviewed URL parameters through WMT and it says it isn't experiencing problems with coverage of the site. The only way I found out about the duplicate title tag issue was through Moz's crawl.
For example:
The main quote page is www.site.com/online-auto-quote
And a variable found would be: www.site.com/online-auto-quote/?location=LondonMoz Crawl is telling me I have a duplicate page title - the URL's look good.
It's the same quote but because the user has indicated they want an auto quote for London when the form is submitted it will send to the appropriate office.

I would look into the Google duplicate content resource.
How are you differentiating between the locations once the user lands on the quotes page? If it's via parameters, then I would make sure that you block those parameters or slap a canonical tag on each one. Do not block the static quotes page URL, you'll want that to be crawled and indexed, I am speaking of variations that populate dynamically.

Do the quote pages need to be indexed? Or can you add a noindex tag to them? This would make it that the quote form would not appear in search engines and alleviate the duplicate title/description issue.
If the page has valuable content on it, of course, this is not the method you'd want to proceed with. But if there are pages on the site that link to the quote page that a user would fill out, then perhaps this could be a good solution?
The other option would be to add a canonical tag to the pages.

How exact should my meta titles and H1 one be compare to the keyword you wish to rank on. Eksample. Is it OK to include name of your town to the title tag or H1 tag on a blog to enhance local search results
I recently attended a webinar by ETNA Interactive on local search SEO. The presenter recommended including the name of your town in the title of the blog to increase local search SEO. Is this OK? Ive always been concerned that it is such an obvious attempt to rank locally that Google would consider it "spammy" ? black hat, "sketchy" or otherwise manipulative. Have the rules changed? Getting Duplicate meta title, and meta description errors
I have a website with shopify CMS. The same type of error (returning a page structure with no content and no CSS) can be replicated by adding a trailing slash to any valid URL. This leads me to believe that there is a problem on the server. It should be serving the custom 404 page instead of junk. I have fixed the "ezine" directory problem by doing a wildcard 301 on that directory, so you can ignore that issue. We are experiencing issues on one of our sites that we cannot resolve. It is an issue that many sites seem to have, according to discussions within the Google Webmaster Tools forum. We have tried several of the suggestions made in that forum, without success. Title Tag Over Optimization
Hey, I've read that adding the company name to the title tag was a waste of space since the more words the less weight each one has, with all this over optimization preventive measures, should I rewrite the title tags format with company names? and if so should it be (company name): (naturally organized keywords) (naturally organized keyword) | (Company name). or can I keep it just naturally organized keywords Also I used to do (keyphrase) - (keyphrase) instead of commas, should I fix this? Is it better to include the secondary keyword or site name in a title tag?
When I add a site name to my title tag with long-tailed primary and secondary keywords the title tag is longer than 70 characters. I need to include all three parts, so what should I do? At 70 characters the site name is usually partially cut off. I do not want to get penalized by Google, but I need to include the site name to have consistency. I am using the format Primary Keyword-Secondary Keyword | Site name

Keyword use in Title tag?
To improve SEO on a particular keyword, should you use that same keyword in the title tag of multiple pages within your site? Will that help or would it actually hurt by causing pages within your site to complete against each other for that keyword? Does it make a difference if that keyword is truly used on all those different pages?
