Is This Worth Fixing?

Hi,
I'm working on a site that was last optimized some years ago. It has a fair number of pages that the url, h1, title tag and image alt exact match. Although this comes back as A+ in Moz's on page grader, it seems a bit much.
What do you think, is all this too heavy an SEO fingerprint for Google?

It's not on the page too many times, it's just relentlessly exact-matched between h1, url, title tag and img alt, but I guess that's okay.

No, if you put the keyword too many times, the page grader will give you a bad grade.

Sounds like it should be fine. Unless Google slaps you for keyword stuffing you haven't done anything wrong...hell it could even be argued you are using a best practice. As long as the URL, H1, title tag, and image are relevant and accurately depict the keyword you are golden.
If anything, you may want to strengthen your keyword strategy and work in some synonyms in there...catch a few more eyes and rank for additional keywords. This can also help to water down the super optimized pages and make it feel a bit safer.

Changing the permalink estructure, worth the risk in 2017 ?
Hello, I have a wordpress blog that have more than 10 years old, when I created the blog the permalink had the date included. Example: site.com/2007/02/02/my-post/ Do you guys think is it worth the risk of changing my url escructure to remove the date ? Ofcourse I would do the 301 redirects and such... What I want to know if this will have any significant SEO advantage considering Google evolved so much ?

Possible Penguin 2.1 fix - Anybody tested this?
Possible Penguin 2.1 fix? This happened to client site - Stay with me - this takes some explaining… A clients home page is set as index.html Which in domain settings goes to the root address: http://www.domain.com/ But is a setting on a domain/hosting - you can set any page to the root- I always link directly to the root address (the second one) So if you set the new root page as http://www.domain.com/index.htm --- going to the root - essentially is a new page- any previous poor linking would be then broken and would have no effect So it would be a matter of changing the domain settings to use the index.htm page (which would function exactly the same- internal link structure of site goes to the root) thoughts?

Should i fix 404 my errors?
We have about 250, 404 errors due to changing alot of page names throughout our site. I've read some articles saying to leave them and eventually they will go away. Normally I would do a 301 redirect. What's the best solution?

Fixing the NAP Interim Idea?
I am realizing that most directory and listings have our old phone number down for our website. And am working on fixing them all. In the interim would it be wise to go ahead and list the old number somewhere on the site (like below the old one on the contact page) or would that create more strings to cut later?

Is it worth re-doing SEO for all existent products
We have a website and when we started, we had no clue about SEO, nor did we really understand the full extent of CRO amongst other things. We have slowly learnt that there are many changes that need to happen to our site; however...do we need to re SEO all the content that is already on the website or can we purely start a fresh with the new products we feed through? Home page deindexed by goole, How to determine why and how to fix
On Wednesday I noticed our domain was no longer ranking for our key word and our product Isolator Fitness, http://isolatorfitness.com, I have been researching and not finding answers to why it happened and what to do to fix it. We have about 800 other pages still listed. I am new to all this seo stuff, can anyone guide me in the right direction.

History about 10 days a go I went google web master tools and noticed that there were a large number of errors due to the fact the robots could not crawl our site. Looked at the site and found that Privacy button on WP was turned to block robots. I turned it on and had google re crawl the site, looked like google was not able to crawl the site for about 3 months, on Monday I did a 301 redirect to on of our other sites for another product we sell, to http://isolatorfitness.com/6-pack-bags. Fixing Google Places once Banned
I have a lot of clients who have somehow botched up their Google Places listing, and now are not showing up in local search results. In one particular case, they were using 2 different Gmail accounts and submitted their listing twice by accident. It appears Google has banned them from local search results. How does one undo steps like this and start fresh? Thanks!
