Unable to Get Back to Performance Levels Six Months After Attack

We work on a fine art site. In May we were attacked and blocked by Google for about 12hrs until we cleared all affected pages and files. Our response was pretty fast, and to be fair, so was Google's action to unblock us.
However, shortly afterwards, the entire site's page rank disappeared, it never came back. I also believe people are now speculating that Google's PR updates are either overdue or have stopped, so I can't gauge PR at all.
One way we can gauge potential for position is that we remain top 1-6 in Bing for most terms. Moz authority reflects this as well. An example:

These are the types of position we occupied on Google prior to the drop. We've been told there are no manual actions against us, nevertheless we've worked to clean out any duplicate content and disavow some link trawling/people directory sites that duplicated a lot of our content. Page code has been cleaned up, pages optimised, loading times reduced where possible etc. etc..

Any input/experience gratefully received. Working our wotsits off to generate traffic off no PR and Adwords at the moment and it's a struggle. I've actually got new sites to rank faster and better than this in 3 months, so this is one big penalty.

A few points.
1. Make absolute sure your site is clear of Malware. Check Google Webmaster Tools and I also recommend running your URL through the Securi checker (the scan is free, and the service is worth it if you were hit)
2. After a malware hit, Google recommends you use the "Crawl as Googlebot" in Google Webmaster Tools and then select "Submit this page and all linked pages to Index" option.
3. Make sure your sitemaps are up to date and properly submitted via Google Webmaster Tools.
4. I did notice in your question that you disavowed links. If you disavowed links that were actually helping you this could have had a negative effect on your rankings. Unfortunately, I don't know of a way to get these back.
It's possible your drop has nothing to do with the Malware - it could be just an unfortunate coincidence. Do a site audit, check your crawl errors, try to match your traffic drop against known Google updates, and stay vigilant about improving your content and outreach.

Is site: a reliable method for getting full list of indexed pages?
The site:domain.com search seems to show less pages than it used to (Google and Bing). It doesn't relate to a specific site but all sites. For example, I will get "page 1 of about 3,000 results" but by the time I've paged through the results it will end and change to "page 24 of 201 results". In that example If I look in GSC it shows 1,932 indexed. Should I now accept the "pages" listed in site: is an unreliable metric?

My website internal pages are not getting cached with latest data
In our website we have sector list, in home page main category list is displayed click on main category user has to select sub category and reach the result page. EX: Agriculture->Agribusiness->Rice Agriculture page is indexed,but Agribusiness and Rice page is not getting cached,it is showing old indexed date as 23 July 2013,but i have submitted the sitemaps after this 4 times, and some url i have submitted manually in web master tool, but after this also my pages are not cached recently, Please suggest the solution and what might be the problem Thank you In Advance, Anne

Sitemap do they get cleared when its a 404
Hi, Sitemap do they get cleared when its a 404. We have a drupal site and a sitemap that has 60K links and i want to know if in these 4 years we deleted 100's of links and do they have them automatically cleared from Sitemap or we need to build the sitemap again? Thanks

No progress in 3 months!!

I have been using Moz for about 3 months now and love the tools and support. I have been working really hard on seo improvements, fixing lots of coding errors and adding content to my site. My last 3 keyword reports have been very disappointing. I have lost rankings in many of my keywords and some have been ranked very well for years. (talking google) I see only 2 factors that have changed. 1. My 'improvements' and 2. the google update. As everyone knows seo is very time consuming and I am so disappointed with my site going backwards after such a dedicated focus. An example would be the keyword 'christmas return address labels'. I am not even ranked in the top 100 and the last report put me down 50! Why do I get duplicate page title errors.
I keep getting duplicate page title errors on www.etraxc.com/ and www.etraxc.com/default.asp, which are both pointing to the same page. How do i resolve this and how bad is it hurting my SEO.

How to get Google to index another page
Hi, I will try to make my question clear, although it is a bit complex. For my site the most important keyword is "Insurance" or at least the danish variation of this. My problem is that Google are'nt indexing my frontpage on this, but are indexing a subpage - www.mydomain.dk/insurance instead of www.mydomain.dk. My link bulding will be to subpages and to my main domain, but i wont be able to get that many links to www.mydomain.dk/insurance. So im interested in making my frontpage the page that is my main page for the keyword insurance, but without just blowing the traffic im getting from the subpage at the moment. Is there any solutions to do this? Thanks in advance.

How long does it take for traffic to bounce back from and accidental robots.txt disallow of root?
We accidentally uploaded a robots.txt disallow root for all agents last Tuesday and did not catch the error until yesterday.. so 6 days total of exposure. Organic traffic is down 20%. Google has since indexed the correct version of the robots.txt file. However, we're still seeing awful titles/descriptions in the SERPs and traffic is not coming back. GWT shows that not many pages were actually removed from the index but we're still seeing drastic rankings decreases. Anyone been through this? Any sort of timeline for a recovery? Much appreciated!
