Https and 404 code that goes into htaccess

The 404 error code we put into htaccess files for our websites does not work correctly for our https site.
We recently changed one of our http sites to https.
When we went to create a 404.html page for it by creating an htaccess folder with the 404 error code in it, once we uploaded the file all of our webpages were displaying incorrectly, as if the css was not attached.
The 404 code we used works successfully for our other 404.html pages for our other sites (www.telfordinc.com/404.html). However, it does not work for the https site.
Below is the 404 error code we are using for our https site (currently not uploaded until pages display correctly)

So we want to know if there is a different 404 error code that goes into the htaccess file for an https vs. http?

My site doesnt give any 404 error
Hi guyz, I've realized that when someone try to access some url that doesn't exist on my site, my site gives a custom 404 page but not give any 404 http status code.

Moz Reporting Incorrect 404's
Hi Guys SEOMoz is telling me that we have 191 404 errors f. I have checked this with several other crawlers and this not the case. For example, http://www.opticalexpress.co.uk/eyecare/corporate-savings.html%0D%0A2027 But correct links its http://www.opticalexpress.co.uk/eyecare/corporate-savings.html which is fine... We have no record of these links so why is it appending these characters at the end of the URL which is causing the 404's....

Duplicate content and http and https
Within my Moz crawl report, I have a ton of duplicate content caused by identical pages due to identical pages of http and https URL's. For example: http://www.bigcompany.com/accomodations https://www.bigcompany.com/accomodations The strange thing is that 99% of these URL's are not sensitive in nature and do not require any security features. No credit card information, booking, or carts. The web developer cannot explain where these extra URL's came from or provide any further information. Advice or suggestions are welcome! 404-like content
A site that I look after is having lots of soft 404 responses for pages that are not 404 at all but unique content pages. the following page is an example: http://www.professionalindemnitynow.com/medical-malpractice-insurance-clinics This page returns a 200 response code, has unique content, but is not getting indexed. Any ideas? To add further information that may well impact your answer, let me explain how this "classic ASP" website performs the SEO Friendly url mapping: All pages within the custom CMS have a unique ID which are referenced with an ?intID=xx parameter. The custom 404.asp file receives a request, looks up the ID to find matching content in the CMS, and then server.transfers the visitor to the correct page. Like I said, the response codes are setup correctly, as far as Firebug can tell me. any thoughts would be most appreciated.
