A good title for each page on my website.

Dear SEOs,
I know these facts:
a) not more than 70 chars
b) relevant to the page subject - probably best keyword at the beginning
My problem - should I rebuild the current structure of my titles which is:
- Main Page
keywords (in my case 4) | domain name - Main Page
- Sub-page - one click from main page
keywords (again same 4) | domain name - Sub Page Name (or as you like Category Name)
- Specific page with an article / content
keywords (again same 4 as on any other) | domain name - category name - title of article same as used in
tag
To be more precise, should this title be changed for following pages:
Sub Page - where really it's page that let's you chose particular subjects (there will be few article for each subjects) so should the keywords be changed to - "chose your category"
Specific page with an article - should the category name - title of article be completely removed and first part of title which at the moment contains same 4 keywords used on main page be replaced with keywords specific for the content ?
To be honest I find those titles at the moment bit confusing and sort of illogical. But since I can't really change anything in the code I would like to know what's the right way before I keep pressing on the programmer

I think a key thing to remember is that you should optimize for users and then search engines, not engines and then users. Using the same 4 keywords in every title is not going to add much benefit to ranking well in Google and it is going to look really funny to users. Also to remember is that that title is what will (hopefully) appear in search results pages. You want to write something that users will click on.
For example, take a look at SEOmoz's homepage "SEO software. Simplified. | SEOmoz" That covers all the bases, includes the main keyword, but in a natural sort of way.
I would definitely use a " | company name" at the end of the title, but I would rethink your use of keywords.

Website Not Indexing
My website is not indexing due to someone's complaint in Google that was registered in 2013. The complaint says I am using copyrighted images of someone else on my website but those images were immediately removed by me. Why is Google replacing my meta title with the business name on home page?
For all queries that return the home page, Google is not showing my meta title. Instead it replaced it with the official business name which of course makes it harder to rank for key terms since they don't exist now in the meta title. You can see this is you search on "mt view estate planning attorney". The site in question is dureelaw.com and the title showing is "The Law Office of Daniel L. DuRee." View the source and you'll see my meta title. Why is Google substituting it?

Moving from Bigcommerce to Woocommerce on WP. Should we redirect size pages into one page?
We are moving from Bigcommerce to Woocommerce on WP. On Bigcommerce, due to some bizarre reasoning the previous developer had 3 separate URLS for the same product in different sizes - S, M and L. Now we plan to have one product page where the sizes can be selected and 301 redirect the 3 urls to the new one. Is this advisable? Or should we just have 3 separate pages. OR should we have one of the sizes pages as the new page and then redirect the other 2 to this one? I ask this because the site has a LOT of ranking power and we do not want to jeopardise that.

Page Not Indexed
Hi Guys I wrote and published an article last night on my site but it is yet to be indexed. This is strange as articles are usually indexed pretty quickly. Different Title Tag and Page Headline
My editorial team won't budge with their headlines which are excruciatingly vague ... But I have managed to convince them to let me optimize the title tags and the URLs. Is this sub-optimal or are there some benefits to having a title tag that varies from the page headline or what our dev team calls the "reader friendly" title? For example... Is it a good idea to rel=canonical dozens of old outdated pages?
we have dozens old outdated manual pages that still need to be up, but have terrible code issues (they're exported from word) and no image tagging, etc. there are new pages in place, so should i rel=canonical to the new pages? will this transfer any link juice to the newer, more seo-friendly ones?

Crawl Diagnostics - Duplicate Content and Duplicate Page Title Errors
I am getting a lot of duplicate content and duplicate page title errors from my crawl analysis. I using volusion and it looks like the photo gallery is causing the duplicate content errors. both are sitting at 231, this shows I have done something wrong...
