What is the best permalink structure for SEO?

Your feedback here is definitely appreciated, but I'm also doing a public study and would be honored and humbled if you answered the 5 questions in my survey as well.
For those who do not wish to participate, I'd appreciate your general feedback on permalink structure best practices based on what Amazon.com and eBay.com have done to their URLs in recent times.

Fascinating. How do we explain eBay and Amazon URLs? Their breadcrumb system and categorization produce Google breadcrumbs and backward navigation, yet the URLs have absolutely no categories/directories.

I think it really depends on the content. Does the user need to know what category or not? Or does the content need to be organized some way. ie. if you have a car site you can have a page /ferrari/ with all models on it with each model having its own url /ferrari/360-modena but for a toppic on that site "best ferrari service in NY" then you dont necessarily need /advice/best-ferrari-service-in-ny I think it just depends on the content and user friendliness.

yep and can make it more user friendly for remembering the url extension and sharing via social media, not to mention for PPC display url.

Also I read somewhere here in the forum that a www.mysite.com/keyword/ would be better than a www.mysite.com/keyword.html because it have less word, so, each one carry more value.

Just answered questions on your survey. I am liking this format these days for long term www.mysite.com/keword or www.mysite.com/keyword/ this makes it best for long term if you decide to switch platforms down the road. Looking back if we built links to all these pages /keyword/ vs keyword.php we wouldnt have to 301 everything in the future if it changes to .html or .htm etc.

Dealing with negative SEO
Interested to know people strategies for detecting and mitigating negative SEO. Previously I've used link monitoring tool and kept an eye on all new back links coming in to any page on the site. I have then manually assessed each one again using some tools and actually visiting the website. However, this always leaves me with one dilemma. Regardless of my assessment how do search engines see that link? I run three lists a white list, grey list and blacklist. White list - very relevant and have a lot of authority. I.e. leading industry blogs and forums. Grey list - out of topic/industry, directories Blacklist - sites de-indexed by Google, illegal content or absolute spam (i.e. one page filled with hundreds of links to different domains) Do you have any thoughts? How do you assess if link is bad?

What's the best SEO practice for having dynamic content on the same URL?
Let's use this example... www.miniclip.com and there's a function to log in... If you're logged in and a cookie checks that you're logged in and you're on page, let's say, www.miniclip.com/racing-games however the banners being displayed would have more call to action and offers on the page when a user is not logged in to entice them to sign up but the URL would still be www.miniclip.com/racing-games if and if not logged in, what would be the best URL practice for this? just do it?

Does capitalization matter for SEO?
Two places capitalization comes into play: (1) on-page use (title, h1, body text, img alt text, etc) (2) external anchor text I didn't think it mattered from Google's point of view for on-page usage (is this correct?) but I notice that OpenSiteExplorer' s 'anchor text distribution' tab shows different counts for the same keyword if it's capitalized in different ways (eg seomoz.org is listed separate from SEOmoz.org). Is that just OSE or does Google treat the keyword/phrase different based on its capitalization, too? And if so, then should I be creating external links to my site with the 'regular' and 'Capitalized' versions of my key phrases?
